4 Risk Management Column I For possible options, follow table on the right. Plumbing Material Compliance
5 Correction Column Q Mention how the problem was fixed. HVAC Equipment Malfunction
6 Corrective action Column R Mention the actions taken to prevent reccurance of the problem. Telecom etc..
7 Days Past Due Column U 1- NCR open no ACD Today's date - Contractual Closing date * Welding
2- NCR open with ACD Today's date - ACD date Piping
3- NCR closed no ACD Closed date - Contractual Closing date * E&I
4- NCR closed with ACD Closed date - ACD date Coating
8 Estimated Cost Column V Cost of Rework Including (Manpower, Material, Repair) HSE
Structural
- All columns in blue are mandatory and should not be left blank, any thing not aplicable should Landscaping
be designated by N/A. etc…
- Any additional site based / Client based information can be added using the yellow
columns on the right.

* Contractual Closing date is the date after the maximum allowed days for an NCR to stay open
as per contract agreement, counting from issue date.
