Dear Doctor,
I am writing to request for an urgent admission for further investigation and treatment for Amina,
whose features are suggestive of meningococcal meningitis.
Amina, who was under my treatment as a case of viral infection for 5 days, visited today with anxious
parents as her condition was progressively deteriorating. She had severe headache, lethargy, listlessness
and vomiting. Examination showed neck stiffness, maculo-pappular rash over the legs, temperature
-40.2*c, pulse – 110/min. Blood report showed leukocytosis with left shift. With all this features I
suspect meningococcal meningitis and started penicillin IV, stat dose of which has been given.
Initially, on 09/10/12 she presented with fever, runny nose, mild cough and loss of appetite and
managed with paracetamol and rest. As her condition did not improve, 2 days ago I instituted Brufen
200mg and ordered for blood and urine tests. All her observations were normal during both visits except
the temperature which were 39.4 *c and 39.8*c accordingly.
Please note that, Amina has no known allergy. She lives with her parents and two younger siblings, aged
4 and 2.Moreover she is school going. Although her family emigrated from Somalia 7 years back they
understand English.
It would be appreciated if you arrange an urgent admission for her assessment and management. If you
