Patient’s family : Good morning nurse
Nurse : Introduce me nurse Yuli. I am the nurse on duty this morning from 07.00
to 14.00 noon. Are you the child of this cared mother?
Patient’s family : Yes, I am her son
Nurse : What’s your name?
Patient’s family : Dinda sus
Nurse : Well, Miss Dinda, we have to fill in the new patient admission form first
and also sign it. Because based on the examnations we have done, you
have to be hospitalized.
Patient’s family : Yes, I understand.
Nurse : Ok, can we start now?
Patient’s family : Yes, nurse
Nurse : You can say your mother’s full name and date of birth
Patient’s family : Ms. Yati Hartati, she was born in Jakarta on July 20, 1985
Nurse : Now where does she live and do you live at the same address?
Patient’s family : Yes, we live together at Sudirman street no. 45 Cijantung, East Jakarta
Nurse : Do you have phone number so we can call you in case of an emergency?
Patient’s family : Yes sure, my phone number is 0813
Nurse : Okay, is she allergic about something, such as food or drug, miss?
Patient’s family : No, nurse
Nurse : He she ever suffered from any disease before?
Patient’s family : No, she hasn’t it
Nurse : Previously, I will give you and your mother an orientation about your
family or relatives waiting for only one person to wait. Then regarding the
facilities in this room, there is a bathroom so that you can shower and
defecate there. The nurse who will take care of your mother is myself and
the doctor in charge is doctor Algi.
Patient’s family : Yes nurse
Nurse : Is there anything you want to ask again miss?
Patient’s family : No nurse
Nurse : Alright miss, this patient acceptance has been completed. If you have
nothing to ask, I want you to sign the new patient admission sheet, miss
Patient’s family : Yes nurse
Nurse and family sign a new patient admission sheet.
Nurse : In 15 minutes I will come here to give medicine to mother. If you or
mother need help, you can call the nurse by pressing the bell or call the
nurse at the nurse station. Excuse me, miss.
Patient’s family : Yes, thank you nurse
Nurse : You’re welcome
