This is to certify that, the parent/guardian of DIETHER F. ENRIQUEZ


has allowed my son/daughter to participate on COMPUTER SYSTEMS
SERVICING NC-II refresher course program in preparation of National
Assessment. In this connection, I allow my son/daughter to attend enhancement
sessions conducted on Thursdays, Saturdays and Sundays only for the month of
January and February second semester school year 2020-2021.
Aware of possible outcome of the activity and allowed my child to
participate on the activity voluntarily without compulsion and claimed no further
charges against the organizers and teachers of the said activity, since
precautionary measures have been done.
