Observed workers are working inside deep excavated area without soil TBT & TAB conducted

. on daily basis
protection precaution.

Observed deep excavated area without soil protection & hard Disinfection spray is spraying at offices, toilets &
barricade. equipment's cabin on daily basis.

Material stocked properly.


Found damaged high voltage cable during excavation work.

Mechanical excavation was in progress without obtaining a Rebar caps installs on protruding rebar's.

Heavy equipment's inspection checklist is carrying on daily


Observed deep excavated area without soil protection & basis.

Observed Foreman/supervisor roaming at site without HSE site walkthrough is in progress for the improvement of
wearing mandatory PPE’s (Safety Helmet & Safety Shoes). HSE culture.

Shoring activity is carrying out without adequate illumination.


Heavy Equipment's & operator have valid documents, TPC
Excavated area itself confined space & No valid permit
insurance etc.
obtained for confined space entry, while heavy equipment’s is
moving upside of excavated area, risk of collapse

Observed, power tools (grinder) was using with 3 prong plug Flagman appointed at road junction to control traffic flow.
along with cable drum, which is strictly prohibited by DGDA

Observed power tool (grinder) used without grip handle at Ventilated confined space area prior to enter
site workplace.

Chemical Buckets kept at non-designate area. Housekeeping is in progress to maintain workplace area.

Observed unsafe lifting, no tag line place, lifting radios area without
barricade, workers are not wearing mandatory PPE’s

Observed unwanted steel plat kept at access road.


Observed deep excavation with incomplete shoring & without edge


protection.

Observed unsafe lifting, lifting is carrying on without


barricade the area, and workers failed to wear mandatory
PPE’s.

Sewerage water manhole is overflowing on the access road.
