Date of Incident: 11.08.2017 Time: 12:45 PM Shift: ‘D’ Location: Dryer Area Department: IMMM (Pellet Plant) What happened: Mr.D.Chatterjee was in General shift (D) duty. At about 12:45 PM after taking lunch he was going to drink water from the near by designated drinking water point. He stepped his foot on the drinking water pipe while approaching towards the water tap. He lost his balance in the process and his ankle got twisted. He was sent to west plant first aid and subsequently referred to TMH emergency for carrying out X-ray. Hair line fracture was detected in his right ankle. Plastering of his right foot was done by the treating doctor at ESI and he has been made unfit for one month.

Preliminary findings:
1. He stepped his foot on the drinking water pipe
2. It was a designated drinking water point
3. One piece of metallic plate was found in the approach path of water point
Recommendations:
1. Immediate counseling done
2. All the employees of the section were communicated of the incident 3. Coaching and counseling of own and contract employee for safe behavior at work place 4. Condition of all drinking water point to be improved after survey. In general they are in good condition but even minor abnormalities to be corrected 5. Housekeeping in the surrounding area to be improved 6. Any trip hazard in the near vicinity to be corrected 7. PIR No. IN00042517 logged for further investigation
