DESCRIPTION OF EMERGENCY SCENARIO

 On the morning of 14 August 2020, as part of a maintenance programme, an effluent pump was removed from the effluent tank, and as the work could not be
completed that day, a blanking plate was fitted over the end of pipe where the pump had been removed. That evening, without the Process Malting Technicians
knowing, and when Technician on shift drained effluent from the steep tank to effluent tank, the effluent escaped through the banking plate into the bundwall.
At around 08.30 the Plant Manager was doing plant walkabouts. He discovered effluent on the ground outside the effluent holding tank bundawall.


RESPONSE EVALUATION

2 Scene Preservation (barricading) The scene was preserved.
3 Emergency response team The response team came onsite and responded well to close the leak and trace the flow of the effluent. Effluent was
flowing into the drains connected to Municipal storm drains. The capacity of the bundwall could be enough to
contain effluent from the tank but it was observed that the bundwall was leaking and the valves on the bundwall
were left open.
4 Ambulance (Mars/EMRAS) response Ambulance was not required in the drill
5 Management/ Supervisors The Plant Engineer, Process Manager and QC Manager came onsite. SHE officer coordinated the emergency
management
6 Overall Response The overall performance was commendable. People knew satisfied with the overall containment systems despite of
the leaks on the bundwall
