Psychosocial Intervention Program

Dear Respondents;

Thank you for your participation in this research regarding physical and

mental readiness to online teaching. Information gathered from this study may

be utilized to craft a Psychosocial Intervention which could be a basis in

providing appropriate programs and resources to help in promoting quality

online instruction. Because of your background and experience, your

participation and input are invaluable.

This Google survey form has three parts: (1) gathering information about

the demographic profile of respondents, (2) teachers’ physical readiness to

online teaching, and (3) teachers’ mental readiness to online teaching.

Your participation is voluntary and uncompensated. No identifying

information is being gathered and your participation is anonymous and

confidential. There are no known benefits or risks to this research. If, at any
