 Instructions
This assessment was created with the intention
of assessing students three times a year:
beginning of year, mid-year, and end of year.

When assessing students, I recommend making

enough copies of the assessment to record
results for each child, as well as one copy to give
to students. Give each student their assessment
sheet so they can write their name and letters.
Then, take the sheet to record their results. Give
students the reference copy of the assessment.
Every student will refer to this same sheet. This
way you aren’t having to mark anything in front
of the child. I feel that marking assessments
correct/incorrect in front of a child sometimes
hinders their focus.

After assessing students, record the results on

the assessment data tracker. The data tracker
is divided into three sections so that you can
easily keep track of progress throughout the
school year. Two assessment data trackers are
included to fit your class size.
