Catatan 1 :
1. AA = Stop operasi, perbaiki segera ; A = Perbaiki dalam 12 jam ; B = Perbaiki dalam 3 hari C
= Perbaiki Jika dapat / mungkin dilakukan
2. Kendaraan dioperasikan oleh Operator dan memiliki SIMPER yang sesuai dan masih berlaku
3. Posisi komponen unit bisa saja beruba rubah, tergantung series/generasi pembuatan.
4. Unit yang digunakan untuk pemeriksaan posisi komponen adalah type D 375, tapi form ini
bisa digunakan untuk unit unit ukuran yang lain.
5. Katagori kondisi : (Kebocoran, rembes, keausan, kendor, hilang, lepas, patah, pecah, retak, berbau, abnormal dll)
6. Untuk menentukan status (kode bahaya), diperlukan pertimbangan dari tingkat kondisi yang ada.
7. Jika terdapat point pemeriksaan pada form, tapi tidak ada pada unit, silahkan tulis saja NA (Non aktif)
