considered the document to be assessed in the year–end SIP evaluation. Documents of the quarterly evaluation shall be compiled and forms part of MOVs for SBM level of practice evaluation. Schools chosen to present in
the District/Cluster Monitoring, Evaluation and Adjustments shall consider only FIVE (5) activities each from Access, Quality and Governance. Presenters shall bring their SIPs. Members of the School Project Team shall
affix their signatures in all the SIPMEA documents.
