CAPABILITY DEVELOPMENT PLAN FOR THREE (3) YEARS

Project: HYPERBARIC OXYGEN CHAMBER

I. Background:

Hyperbaric medicine is currently gaining attention in the field of medicine with


applications beyond its well-established definitive treatment for decompression
sickness associated with scuba diving accidents including arterial blood embolism
due to pulmonary barotrauma from ascent. The treatment comprises hyperbaric
oxygen therapy (HBOT), which involves breathing pure oxygen at an ambient
pressure higher than atmospheric pressure, and therapeutic
recompression for decompression illness intended to reduce the injurious effects of
systemic gas bubbles by physically reducing their size and providing improved
conditions for elimination of bubbles and excess dissolved gas. New applications for
HBOT includes serious infections, non-healing wounds associated with diabetes or
radiation therapy. Proposed mechanisms of action include increase oxygen supply to
areas of injury, stimulate capillary growth, create immediate aerobic state, release of
growth factors and stem cells which promote healing.

The need for Hyperbaric medicine chamber was first conceptualized in one of
the visits of PRESIDENT RODRIGO R. DUTERTE to AFP Health Service Command
last 02 August 2016. It was determined to be an important intervention in
enhancement of wound healing of battle casualty patients. Last 21 December 2017,
the Armed Forces of the Philippines represented by the Secretary of National
Defense DELFIN N. LORENZANA and the winning bidder Joint Venture of Variance
Trading and Cartal Enterprises Inc. entered into a contract Agreement for the
Acquisition of One(1) Lot Hyperbaric Chamber for the Armed Forces of the
Philippines Health Service Command amounting to Eight Million Three Hundred
Eighty Thousand pesos only(PhP88,380,000.00). (DND/AFPMC-ME-HC-R17-0005).

II. Scope of Utilization

The US FDA and Undersea Hyperbaric Medicine Society (UHMS) has 14


approved HBOT indications similar with U.K. For other countries, Japan has 20,
China 53, Canada, Germany and Russia 85. The Philippines has no training,
certifying, nor regulating agency for the practice of Hyperbaric Medicine.

Approved Indications
Decompression sickness Diabetic wound healing problems
Air and Gas embolism Crush injuries, Compartment syndrome,
Acute ischemia (traumatic or vascular basis)
Carbon Monoxide poisoning or smoke Compromised skin grafts and flaps
inhalation
Clostridial myonecrosis (gas gangrene) and Severe anemia (due to blood loss)
anaerobic infections
Delayed radiation injuries Refractory osteomyelitis
Osteoradionecrosis Acute hearing loss. Acute acoustic trauma
with or without tinnitus

Thermal burns (2nd degree or more) Necrotizing otitis externa

Other indications considered optional (AHLSI-Philippines) include


enhancement in healing of certain wounds, post anoxic encephalopathy, Acute
ophthalmologic ischemia, Radiation Necrosis (intestinal and other).

Experimental indications include Cerebral Palsy, Stroke (Acute cerebral


ischemia, Lyme disease and autism.

III. The Hyperbaric Medicine Program for VLMC

SUPPORT OF THE VLMC AND THE AFPHSC IN GENERAL”

IV. Organizational

The Department of Hyperbaric Medicine is proposed to be under the


Medical Service Division among the different clinical departments. Its
organizational structure as a department shall be headed by a Chief supported
by a Safety Director and the different personnel representing the physicians,
nurses, and technologies needed in the applications and operations of the
chamber.
