AFFIDAVIT OF DISCREPANCY

I, VICTORIA F. ARQUELADA, of legal age, single, Filipino and a resident of Brgy.


San Pedro, Vigan City, Philippines, having been duly sworn in accordance with law,
hereby depose and state:

1. That I am the sister of the late GERARDO F. ARQUELADA who was born on
September 07, 1957 and joined his creator on January 01, 2018;

2. That there is clerical or typographical error on his pertinent documents such as in


his Voter Certification, PhilHealth Member Data Record and other Government
issued Identification Cards because his date of birth appearing therein is
September 07, 1956 instead of September 07, 1957 which is his real date of
birth:

3. That in the absence of Certificate of Live Birth, his Elementary Pupil’s Permanent
Record at Mindoro-San Pedro Community School, it is stated that he was born
on 07 September 1957 at San Pedro, Vigan City, Philippines;

4. That in his honest belief that he was born on September 07, 1956, he used this
date as his date of birth in his pertinent documents during his lifetime;

5. That I am executing this Affidavit of Discrepancy to attest


to the truth of the foregoing facts and to explain the discrepancy in my brother’s
pertinent documents such as in Voter Certification, PhilHealth Member Data
Record, Student Permanent Records, Voter Certification and other Government
issued Identification Cards as regards to his date of birth;

6. Finally, I am executing this affidavit to attest to the truth of the foregoing and for
all legal intents and purposes this may be worth.

IN WITNESS WHEREOF, I have hereunto set my hand this 8th day of February,
2018, at Vigan City, Ilocos Sur, Philippines.

I, CLARA F. ARQUELADA, of legal age, single, Filipino and a resident of


Barangay San Pedro, Ilocos Sur, Philippines, having been duly sworn in accordance
with law, hereby depose and state:

That my name is CLARA ARQUILADA as stated and registered in my birth


certificate issued by Office of the Civil Registrar of Vigan, Ilocos Sur. A copy of my birth
certificate is hereto attached as “Annex A”;

That since childhood, I have been accustomed to using the surname


ARQUELADA in my personal records, transactions and communications; A photocopy
of my valid identification documents is hereto attached as “Annex B”;

That the surname ARQUELADA and ARQUILADA refer to one and the same
person, the herein surname of the affiant;

That I am executing this Affidavit to attest to the truth of the foregoing facts and
to use the same for whatever legal purpose it may serve.
