Dear Sir/Ma’am:

The Schools Division Office through the Curriculum Implementation Division will conduct
a Division Demonstration Teaching Festival 2021 in which certain teachers at Caloocan City
Science High School will participate. As one of the participating teachers in the Senior High School
Department, I am requesting your child to be present on May 14, 2021 from 2:00 PM to 3:00 PM
for my pre-recorded demonstration teaching via Google Meet. Furthermore, we are asking for your
consent to:

 Record your child’s name, image and voice during the demonstration teaching, and any
personally identifiable information shared by your child during demonstration teaching.
 Share the recording of the demonstration teaching with School Head, Master Teachers,
Subject Coordinator and Public Schools District Supervisor-in-Charge for the selection
process.
 Include the recording of the demonstration teaching in the compendium of recorded
demonstration video lessons to supplement teaching and learning resources.

Thank you very much for your participation and we are hoping for your positive response on
this matter.

Consent Clause: I hereby consent to Caloocan City Science High School the recording of my
child's name, image and voice during participation in demonstration teaching through Google Meet,
as well as any personally identifiable information my child may share during instructional activities;
agree to the subsequent release of such recording School Head, Master Teachers, Subject
Coordinator and Public Schools District Supervisor-in-Charge for the selection process; and agree to
the inclusion of the recording of the demonstration teaching in the compendium of recorded
demonstration video lessons to supplement teaching and learning resources.
