1) The complainant was distributing food packs in Barangay [PLACE] on November 1, 2020 around 5:00 PM along with colleagues. 2) While distributing the food, the respondent appeared on a motorcycle with two others and threatened the complainant, saying "Daputa ka, Daputa Kamo, Patyon ta ikaw". 3) Due to the willful and felonious threats, the complainant suffered mental anguish, fear, anxiety and damage to their reputation. A colleague witnessed the incident and will provide a supporting affidavit.

COMPLAINT-AFFIDAVIT

I ________________, of legal age, Filipino citizen, married, a


resident of_________________________, after having been duly sworn to
in accordance with law, do hereby depose and say that:

1. That I am currently employed


at_____________________________;

2. That on November 1, 2020, at around 5:00 in the afternoon we


were distributing food packs to the residents of of
Barangay_______________.

3. On the same day,(November 1, 2020) while we were distributing


food packs to each household of Barangay
___________________the herein respondent
______________________on board a motorcycle with his two
colleagues suddenly appeared in front of us and uttered the words
“Daputa ka, Daputa Kamo, Patyon ta ikaw”.

4. That due to the said willful, unlawful and felonious threat inflicted
by the respondent, I, suffered and continue to suffer serious
anxiety,mental anguish, sleepless nights, fear and trauma not to
mention the damage they have caused to my reputation among my
colleagues and the general public who witnessed the incident.

5. Additionally, All this incident was witnessed by my colleague


___________ who will execute an affidavit to this effect and herein
marked as “ANNEX ___ ”.

6. I am executing this affidavit to attest the truth of the foregoing


statements.
