I, MELISSA FERNANDO GERMINO, of legal age, married and


currently residing at #73 Unit I, Cenacle Drive Sanville Subdivision,
Barangay Culiat, Quezon City, subscribing under oath in accordance with
law, depose and state that:

1. I am the petitioner in the above-captioned case; and I caused


the preparation and filing of the foregoing Petition; and I have read and
understood the contents thereof and the same are true and correct of my
own personal knowledge and based on authentic records;

2. That the petition is not filed to harass, cause unnecessary delay


or needlessly increase the cost of litigation; and

3. The factual allegations therein have evidentiary support or, if


specifically so identified will likewise have evidentiary support after a
reasonable opportunity for discovery;

4. I have not therefore commenced any other action or filed any


claim involving the same issues in any court, tribunal or quasi-judicial
