AFFIDAVIT

I, PSSg Rodolfo G Pacaro, legal age, married and a bona-fide member of PNP designated
as and presently assigned at the San Miguel Municipal Police Station, San Miguel, Zamboanga
del Sur in accordance with the law do hereby depose and state that;

1. That, I was one of the personnel detailed to conduct Law Enforcement Operation
particularly service of warrant of arrest to the suspect identified as Albert Matchuca on
February 9, 2021 at about 9:30 am at Purok 2 , Barangay Dumalian, San Miguel, Zamboanga
del Sur;

2. That, during the service of warrant of arrest while proceeding to the subject’s
residence, I accidentally stepped on a slippery rock resulting to loss of balance. I fell to the
ground and unconsciously hit the hard rock with my right upper legs where my Short Firearm
was placed. My Team Leader PMSg Belbert Intel PNCO who was at my back immediately
assisted me to recover and we noticed that my issued Short Firearm (Beretta with Serial Number
Mo9753Z) was already destroyed and divided into parts;

3. That after the Operation, I reported the Incident to the duty Desk Officer to entry the
matter to their Police Blotter and I also informed the Supply PNCO of our Unit PSSg Josepiano
Paningsoro regarding the incident;

That I am executing this Affidavit to attest the veracity of the foregoing facts in support
of any legal action instituted regarding this matter;

I, PMSg Belbert L Orgada , legal age, married and a bona-fide member of PNP
designated Intel PNCO as and presently assigned at the San Miguel Municipal Police Station,
San Miguel, Zamboanga del Sur in accordance with the law do hereby depose and state that;

1. That, I was the Team Leader of the personnel detailed to conduct service of warrant of
arrest on February 9,2021 at about 9:30 AM at Purok 2, Barangay Dumalian,San Miguel
Zamboanga del Sur;

2. Since PSSg Pacaro was assigned here as assistant Team Leader, I find him to be a
responsible Police Officer particularly when it comes to taking care of his issued equipment;

3. That, while proceeding to the residence of the suspect identified as Albert Matchuca ,
I personally saw PSSg Pacaro fell to the ground after stepping to a slippery rock thereafter I
assisted him to recover and I personally saw his damaged Short Firearm divided into parts;

4. That, after the operation I advised him to report the Incident to the Duty Desk Officer
for record purposes and we also reported the incident to our supply PNCO;

That I am executing this Affidavit to attest the veracity of the foregoing facts in support
of any legal action instituted regarding this matter;

I, PSSg Josepiano G Paningsoro Jr, legal age, married and a bona-fide member of PNP
designated as Supply PNCO and presently assigned at San Miguel Municipal police Station,
Sdan Miguel, Zamboanga del Sur in accordance with the law do hereby depose and state that;

1. That, I was the designated as Supply/Logistics Police Non-Commisioned Officer


(PNCO) of San Miguel Municipal Police Station, San Miguel Zamboanga del Sur;

2. That, after the Law Enforcement Operation conducted by our Unit led by, PMSg
Belbert Orgada at Purok 2 Barangay Dumalian,San Miguel Zamboanga del Sur, PSSg Rodolfo
Pacaro informed me that he accidentally stepped on a slippery rock resulting to loss of balance
then he was suddenly fell to the ground and hitting the hard rock with his right upper legs where
his Short Firearm was placed. Accordingly, Team Leader, PMSg Belbert Orgada immediately
assisted him to recover and they noticed that his issued Short Firearm (Beretta with Serial
Number Mo9753Z) was already destroyed and divided into parts;

3. That after informing me the Incident, he showed me the actual condition of his issued
Short Firearm;

4. That I am executing this Affidavit to attest the veracity of the foregoing facts in
support of any legal action instituted regarding this matter;
