As of August 20, 2014, candidates who obtained their pharmacy


education outside of Canada (or the United States) will be required to
register on the Pharmacists’ Gateway Canada and receive a National

IdentiXer Number before applying to PEBC for Document Evaluation


[This does not apply to candidates whose applications for Document
Evaluation were received by PEBC prior to August 20, 2014]. PEBC will
not be able to process applications for Document Evaluation unless a
National IdentiXer Number is provided.

You must apply online. There are two separate applications and two
separate fees for Document Evaluation and the Evaluating
Examination.

Once we have received your completed Document Evaluation


application and fee it will take approximately 6 weeks for an
acknowledgement email to be sent to you. This email will inform you of
the status of all documents required for Document Evaluation. We do
not acknowledge receipt of any other documents until we receive your
Document Evaluation application and fee. If you are concerned with the
delivery status of any of your documents, you should check with the
courier.

PEBC will primarily communicate by email. Be sure to check your email


regularly and make sure your email settings will allow PEBC emails to
reach your inbox.

You will need to submit the following to be considered for Document


Evaluation:

1. Document Evaluation printed application form


2. Full fee (see Schedule of Fees and Fee Instructions)
3. Documents to support identity

Valid (unexpired) Passport;

Birth CertiXcate; or

Canadian Citizenship Card

Marriage CertiXcate or Change of Name CertiXcate (if necessary); or

Statutory Declaration + supporting documents

the application. Incorrectly submitted documents will delay the
evaluation of your qualiXcation.

How To Properly Certify a Document

If PEBC requests that you submit a properly certiXed copy


of a document you must have a copy of the original

If a required document is not written in French or English,


you will need to follow these guidelines to submit a
translation:

1. The translation must be prepared by a government


appointed, ogcial translator.

2. Each page of the translation must include the original


stamp and signature of your ogcial translator.

3. A copy, or original version of the original language


document must be submitted with the translation and
each page must include the original stamp and
signature of your ogcial translator.

4. The name and credentials of your translator must


appear in English or French.

5. The version of the translation must be the original.


Copies are not accepted, even if certiXed.

i. If you are submitting a translation of your transcript, or


licensing statement, the original language version you
send must match the original language version sent
directly to PEBC by your university or licensing
authority.

Form

Candidates must pay by credit card, using the online


application, except in the following circumstances:

You have money on account with PEBC

You are unable to pay online using Visa or Mastercard


credit card

In such circumstances, please


email pebcinfo@pebc.ca for speciXc application
instructions.

Follow these steps for the online application:

1. Enter your name on your application exactly as it


appears on your identiXcation.

2. Fill out all other information requested being sure not


to miss anything as it is all required.

3. Make the required payment online and print the


application form and checklist.

4. You will need two, identical, passport-acceptable


photographs (with date taken, stamped or written on
back by the photographer). They must have been taken
within the last twelve
months. They cannot be photocopies, digital prints or
alter your appearance in any way and they must be
identical photographs.

Attach the photos to the application as the instructions


on the application indicate.

5. You will need to Xnd one of the following acceptable


witnesses:

Notary Public*

Commissioner for Oaths*

Lawyer**

Embassy/Consulate ORcial*
*Stamp/seal and signature required

6. While in the presence of your witness:

Sign the printed application yourself in the space


provided.

Have your witness Xll in city, date, name, signature and


title/profession.

If the required license information or stamp/seal is


not provided, the witness will be deemed
unacceptable, regardless of their qualiXcations.

Have your witness seal, stamp or sign the front of the


photo glued to the 1st page of the application. The
signature needs to cover part of the photo and the
application.

Authorization

If you wish someone to act on your behalf, please Xll out,


sign and send the Authorization Statement, Form 200.
Please note: We need your original signature on this form
so it must be sent by mail.

Instructions For Submitting Necessary IdentiEcation

PRIMARY IDENTIFICATION: We require a properly


certiXed copy of one of the following showing your name
exactly as it appears on your application:

Your valid (unexpired) passport

Your birth certiXcate

Both sides of your Canadian Citizenship Card (only if it


shows all names, including middle names)

* DO NOT SUBMIT your Permanent Resident Card as


primary identiXcation.

IF YOUR NAME HAS CHANGED we require a properly


certiXed copy of one of the following for each name

Your change of name certiXcate

If you have been married more than once, you will need to
submit marriage or divorce papers for each marriage.
Each document must show your surname before and
after the marriage so we can track your names from one
document to the next.

THE NAME(S) ON YOUR DEGREE CERTIFICATE,


TRANSCRIPT AND/OR LICENSING STATEMENT ARE
DIFFERENT THAN YOUR LEGAL NAME AS IT APPEARS
ON YOUR APPLICATION:

You will need to make a statutory declaration before a


Notary Public, Commissioner for Oaths, a lawyer,
or Embassy/Consulate Ogcial that states:

The ogcial name your statutory declaration is


representing

Your date of birth

An explanation of the reason for having to submit a


statutory declaration instead of, or as well as, a primary
piece of identiXcation.

You must submit the original, statutory declaration. PEBC


does not accept copies of statutory declarations, even if
they are certiXed.

When submitting the declaration, certiTed photocopies of


two ogcial documents showing your legal name (with
exactly the same spelling) and date of birth are also
required. Acceptable documents include a permanent
resident card (only if it shows all names, including
middles names and they are not cut off), Canadian
driver’s licence, health card, or secondary school
certiXcate.

You will need to send us a certiXed copy of your


university degree certiXcate with your application

It must be a copy of the certiXcate that you would


frame and hang on your wall

We will not accept a copy of the certiXcate that does


not include the stamp of the individual certifying it

Instructions For University Transcripts

We require your university transcript for your pharmacy


degree program sent to PEBC directly from your
university or aRliated college, either by mail/courier or
email.

Your transcript is a list of all the courses in your degree


program with corresponding grades

The issue date of your transcript must be within one


month of the date we receive it

If you have a Diploma in Pharmacy or another degree


that contributed to your pharmacy degree, you must
also have the transcript for that program sent directly
to PEBC by the issuing institution.

Mailed/Couriered Transcripts:

If your university or college will not send it themselves,


you may have a courier pick up your transcript directly
from the university or college and deliver it to PEBC; the
sender’s address must be that of the university or
college.

We will not accept your transcript if it has been given


to you or anyone else Trst, or if documents that are
not from your university are sent with it. Do not send
your transcript with your application as it will not be

Emailed Transcripts:

We will not accept your digital documents if they were


not emailed directly by the university or an agliated
college.

PEBC reserves the right to conduct a source veriTcation


of your transcript by directly contacting the issuing
university to conTrm the information on the transcript.

Germany:
We require certiXed copies of the certiXcates of all three
stages issued by the University and a certiXed copy of the
pharmaceutical examination certiXcate with attached
original, ogcial translations

Instructions for Licensing Statements

IF YOU ARE CURRENTLY LICENSED

We require a licensing statement be sent to PEBC


directly from all licensing authorities by which you are
currently licensed, by mail/courier or email.

Your licensing statement is a letter stating you are


currently licensed and in good standing

The issue date of your licensing statement must be


within one month of the date we receive it

PLEASE NOTE licensing statements expire 6 months


after their date of issue. If your Document Evaluation is
not completed within this 6-month period, you will be
required to have a new, currently dated licensing

IRAN – Medical Council
ISRAEL – Ministry of Health
SYRIA – Ministry of Health
JORDAN – Jordan Pharmacists Association
EGYPT – The Syndicate of Pharmacists – do not have
documents sent from The Ministry of Health

IF YOU HAVE BEEN PREVIOUSLY LICENSED BUT ARE


NOT CURRENTLY LICENSED:

You must submit a statutory declaration made before a

currently licensed anywhere in the world. It must also
state the country(ies) and name(s) of the
licensing authority(ies) by which you have been
previously licensed. It must clearly state whether
your license was ever suspended or revoked or whether
you were ever the subject of any
disciplinary proceedings. You can send this document
to PEBC yourself. Please make sure it is the
original statutory declaration. PEBC does not accept
copies of statutory declarations, even if certiXed.

You must ask all licensing authorities by which you


were licensed in the past to send to PEBC a statement
indicating that you were in good standing while you
were licensed. It can be sent by mail/courier or email.

We require that this statement be sent to PEBC directly


from all licensing authorities by which you were
licensed in the past.

Mailed/Couriered Licensing Statements:

If your licensing authority will not send it themselves,


you may have a courier pick up the statement directly

you have never been licensed anywhere in the world and
provides a brief explanation of the reason. You can send
this document to PEBC yourself. Please make sure it is
the original statutory declaration. PEBC does not accept
copies of statutory declarations, even if certiXed.

Instruction For Submitting a Syllabus ** Only If


Requested By PEBC**

You do NOT need to submit a syllabus with your initial


application for Document Evaluation

If the university where you obtained your degree has


not already been approved by PEBC, you will be asked
to submit an ogcial university syllabus from the year
you graduated

A syllabus lists all of the courses in your degree


program and provides a description of the course
content

You will need to make sure that the course descriptions


you provide cover all of the courses in your program

You can send the syllabus to PEBC yourself. It does not


need to come directly from the university and it does
not need to be certiXed

Checklist & Mailing Instructions

A checklist will be printed along with your application that


you will need to sign and include with your application.
Please go through it carefully in order to double check
that you are including all of the information required. If all
of these instructions are not followed we may have to
ask for documents to be resubmitted which could cause
you to miss the deadline.

Document Evaluation Results

Once we have received all of the necessary documents your Xle will be
considered complete and, from that date, it will take up to 8 weeks for
your Document Evaluation to be completed. Once it has been
completed a results letter will be emailed to you.

THE FEE FOR DOCUMENT EVALUATION IS NOT REFUNDABLE ONCE


YOUR DOCUMENT EVALUATION HAS BEEN COMPLETED, even if the
outcome is unfavourable and you are determined to be ineligible to
proceed with the Pharmacist Evaluating Examination.

Express Entry for Federal Skilled Worker Program


(FSWP) Applicants and Canadian Experience Class

In addition to applying for Document Evaluation,


applicants under the “Express Entry for FSWP and
Canadian Experience Class” must complete the Request
Form for an Educational Credential Assessment (ECA)
Report. On behalf of Citizenship and Immigration Canada
(CIC), PEBC is responsible for assessing the authenticity
of the primary pharmacist qualiXcation – the candidate’s
pharmacy degree. After PEBC has completed the
Document Evaluation process, PEBC will generate an ECA
Report which will be emailed directly to the candidate. It
is the candidate’s responsibility to submit the ECA Report
to CIC, since CIC only accepts original assessments
directly from applicants.

For more information on the “Educational Credential


Assessment (ECA) for Canadian Immigration Programs:
Express Entry for Federal Skilled Worker Program
(FSWP) and Canadian Experience Class”, refer to the CIC
website.

If you have already passed Document Evaluation and


require the ECA Report you will need to submit a written
request to PEBC with the processing fee.
