STATUTORY DECLARATION

I, MARIA MAKILING, 43 years of age, Filipino, married and presently residing


at 19 Rue de la Fraternité, 94320 Thiais, France, do solemnly and sincerely declare:

1. That I, together with Jose Makiling, and Pedra Makiling, are travelling to
Philippines on 30 April 2021 carrying the following travel
document/passport:

2. That we will stay in the Philippines for two (2) months for the reasons that I
need to accompany my two other children, Angie (18 y/o) and Ana Marie
(16 y/o), (i) to renew/apply for their passport at the Department of Foreign
Affairs (DFA), (ii) to process their visa application under regroupement
familial to the Embassy of France in Makati, Philippines [Ambassade de
France aux Philippines], and, (iii) to travel with us back to France.

3. That we have no signs and symptoms of COVID-19 and we are attaching


our negative result of the COVID RT-PCR test taken less than 3 days
before our departure.

4. I acknowledge that this declaration is true and correct and I make it in the
belief that a person making a false declaration is liable to the penalties of
perjury.
