RESOLUTION AUTHORIZING THE DEVELOPMENT BANK OF THE PHILIPPINES, DAGUPAN


BRANCH AS DEPOSITORY BANK FOR THE SK FUNDS OF BARANGAY ___________ OF
MALASIQUI, PANGASINAN AND GIVING AUTHORITY TO THE SK CHAIRMAN, _________
AND SK TREASURER, _____________ TO OPEN A BANK ACCOUNT (CURRENT ACCOUNT),
TO ISSUE AND ENCASH CHECKS, MAKE DEPOSIT AND NECESSARY WITHDRAWALS AND
OTHER BANK TRANSACTIONS WITH THE DEVELOPMENT BANK OF THE PHILIIPINES,
DAGUPAN CITY BRANCH.

WHEREAS, in accordance with SK Reform Act of 2015 (RA No. 10742), ten percent
(10%) of the general fund of the barangay shall be set aside for the SK. The Sangguniang
Barangay shall appropriate the SK funds, which shall be disbursed solely for youth
development and empowerment purposes.

WHEREAS, under the same law, it provides that the SK shall have financial
independence in its operations, disbursements, and encashment of their fund, income and
expenditures.

WHEREAS, the SK shall open an a current account in the name of the SK with the
Development Bank of the Philippines, Dagupan Branch, where the SK funds shall be
deposited.

WHEREAS, the SK of ___________________ unanimously agreed to open its current


account with the Development Bank of the Philippines where the SK funds will be deposited,
with the SK chairperson and the SK treasurer as the official signatories.
WHEREFORE, on motion of __________ duly seconded by all the members present,
it was

RESOLVED, AS IT IS HEREBY RESOLVED, to open a bank account, to issue and encash


checks, make deposit and necessary withdrawal and other bank transactions with the
Development Bank of the Philippines, Dagupan Branch

RESOLVED FURTHER, that SK Chairman and SK Treasurer, _______________ and


_____________, respectively are the authorized signatories to the current deposit account
to be opened with DBP for the SK Funds. .

RESOLVED FINALLY, to furnish copies of this Resolution to the Development Bank of


the Philippines thru its Dagupan Branch for its reference and guidance.
