2. ORGANIZING SCHOOL PLANNING TEAM
 The team was able to organize SPT with their representations
3. GATHERING OF SPT FOR ORIENTATION, VISION SHARING AND SCHEDULING

The team was able to accomplish the following:

 List of SPT Roles and Responsibilities
 SPT Time Table
4. PHASE 1 – ASSESS

The team was able to document and discuss the initial list of improvement areas

 Accomplish GAP Analysis Template


 Identify PIA’s
 Accomplish first column of the Planning Worksheet
 Accomplish Objectives and Year columns in the Planning Worksheet
 Formulate SPT list of roles and responsibilities or Team Charter
 List possible factors affecting the assigned PIA
 Document FGDs, interviews, or home visits
 Develop a flowchart of the school processes relevant to each PIA
 Document interviews and observations
 Formulate problem statement based on area of focus
 Show diagram fort the root cause
5. PHASE 2 – PLAN

The team was able to accomplish the following:

 List of possible solutions


 Project work plan and budget matrix
 SIP with signatures of SPT members
 Organize AIP Template
6. PHASE 3 – ACT

The team was able to accomplish the following:

 Data from testing the solutions


 Communication plan to concerned stakeholders
 Training plan and other interventions to roll out your solutions
 Plan to gather feedback from concerned stakeholders
 Project Monitoring Report Form
 Updated SRC based on results and reports
 Accomplish SRC
