AFFIDAVIT SOLO PARENT


I, SOCORRO CALLUENG y Diaz, 43 years old, Filipino, single and a resident of
Brgy. San Francisco, San Fernando City, La Union, Philippines, after having been duly sworn to
in accordance with law hereby depose and state it:

1. I am a single mother with two children namely: MERVIN FLOR (15 years old) and
ANGELO FLOR (10 years old);

2. I am presently in custody of ANGELO FLOR while MERVIN FLOR is with their father,
MELVIN FLOR;

3. I and MELVIN FLOR were never married and I remained as single until the present;

4. I am solely responsible for all the needs of ANGELO FLOR including his education;

5. I am executing this affidavit to attest to the truth of the foregoing and for whatever legal
purpose it may serve.

IN WITNESS WHEREOF, I have hereunto set my hand this June 3, 2019 in San
Fernando City, La Union.
