Monitoring of the Extension Activity entitled: “Love your Heart, Call for Your Brain: Webinar on the
Prevention of Stroke & Cardiovascular Diseases”
Description of the Project
In this research, the effectiveness of the said webinar toward the beneficiaries would be examined in
order to draw conclusions. Once information gathered was evaluated, the researchers will then look for
the possible actions or solutions that can be done to help further its beneficiaries. One of these solutions
could be another project or webinar that would focus on filling the shortcomings or enhancing the
effectiveness of the said webinar.

 I confirm that my participation in this research is voluntary.


 I understand that I will not receive any payments for participating in this research interview.
 I understand that most interviewees will find the discussion interesting and thought-provoking. I
have the right to decline to answer any question or to end the interview.
 I confirm that the research interview will last approximately 15 -25 minutes.
 I understand that the researcher will not identify by name in any reports using information obtained
from this interview and that my confidentiality as a participant in this study will remain secure.
 I have read and understood the information provided to me.
 I have been given a copy of the consent form.
 I wish to review the notes, transcripts or data collected during the research interview.
