1. Create a Drug Study on Gastrointestinal Drugs that are commonly used in clinical settings.

a. A neighbor, Mr. Alexander Tofer, is coming to get advice on antacids. He claims he's been
taking Maalox "for years" for indigestion, but it doesn't help anymore. He asks the nurse,
"Can you prescribe another antacid or one of the pricey, fancy pills that the pharmacy sells?
Or should I just take some baking soda? "How is the nurse going to respond?
- Explain to the patient that we nurses cannot give you a drug without a doctor's order,
but I will let you know that one of the most common reasons that patients with GERD do
not react to PPI therapy is because they are not complying with the medication.
According to several studies, only around half of patients are taking their PPIs correctly
at the end of one month. As a result, PPIs are more successful than regurgitation in
treating pyrosis or substernal burning.
- Give the patient the right to medication, dose, and education; by delivering omeprazole
capsules, you must educate the patient. To reduce the danger of aspiration, place the
patient in a side-lying or upright position. To make swallowing easier and promote
medicine absorption and dissolution, offer a glass of water or another oral fluid that is not
contraindicated with the medication, taking into mind any fluid limits. Before signing that
you delivered the drug, stay with the patient until all of the medication has been

consumed.
3. Case Study:
Amanda, 56 years of age, has been self-treated with heartburn antacids for 6 months. GERD has
been diagnosed following an endoscopy of the upper gastrointestinal tract. Decisions were taken to
start treatment with cimetidine (Tagamet. Amanda was generally healthy except for a history of
asthma. She says she doesn't smoke, but she enjoys going to a bingo session every Saturday for a
couple of hours where she's smoking, beer and pizza.
a. What is GERD?
- GERD (gastroesophageal reflux disease, often known as chronic acid reflux) is a condition in
which acid-containing contents from your stomach flow back into your esophagus on a regular
basis. When food reaches your stomach, a valve at the end of your esophagus called the lower
esophageal sphincter fails to seal properly, causing acid reflux. Acid backwash then travels up
your esophagus, through your throat, and into your mouth, leaving you with a sour taste.
b. When Amanda sees the cimetidine medication, she questions, "Why do I need a prescription?
I can buy it from the counter! "What are you going to say in reply?
- As a nurse, I will provide information about the said drug. Cimetidine is an acid reducer
active ingredient available in over-the-counter (OTC) and this drug can also be obtained
without a prescription. It's used to treat heartburn that's caused by too much stomach acid (also
called acid indigestion or sour stomach).
- CIMETIDINE is an antihistamine that prevents stomach acid from being released. It's used to
treat ulcers in the stomach or intestines. It can help with ulcer pain and discomfort, as well as
acid reflux heartburn. Cimetidine is part of a class of medications known as H2 blockers. It
works by lowering the acid levels in your stomach. It's used to treat heartburn that's caused by
too much stomach acid (also called acid indigestion or sour stomach).
d. Develop a nursing care plan (minimum 1 NCP) for Amanda. You may use NANDA Book as
your reference.

- Determine an appropriate body weight for your age and height. For a thorough nutritional assessment and
nutritional support options, consult a nutritionist.

- Create a comfortable atmosphere.
- Encourage correct positioning.
Planning:
As demonstrated by the following signs, the client will consume daily dietary requirements in accordance
with activity level and metabolic needs:
- Discuss the significance of proper diet.
- Determine if your daily consumption is deficient.
- Relate appetite-inducing techniques.

Intervention:
- Enhance diet
- Establish a dietary plan for weekly weight loss goals of one pound
- encourage patient to make gradual changes in dietary habits; encourage small frequent meals of high
calories and high protein foods
- instruct patient to eat slowly and masticate foods well
- provide activities for the patient that are not centered around or associated with meals or snacks.
Evaluation:
Goals are met as evidenced by:
- The patient will consume everyday nutritional requirements based on his activity level and metabolic
requires.
-The client will be able to value the importance of proper nutrition.
- The client will have a better understanding of the actions she can take to improve her nutritional diet.
- The client will accomplish and maintain a healthy body weight.
