4. We are executing this affidavit to the best of our knowledge and


belief to substantiate our Marriage which we could not be obtained
from the Office of the Municipal Civil Registrar of
________________due to the fact that after a thorough and diligent
search made on the existing records of the said office, the record of
our birth does not appear therein;

5. We are executing this Affidavit to the best of our knowledge and in


good faith, and for whatever legal purpose this may serve.
