I, JOSE VICENTE A. AGUILA, of legal age, Filipino, with principal office


address at 8480 Km. 18, East Service Road, Brgy. San Martin de Porres, Paranaque City,
after having been duly sworn to in accordance with law, do hereby depose and say:

1. That I am the Corporate Secretary of COMGLASCO AGUILA GLASS


CORPORATION;

2. That the DAVAO DIVERSION branch of Comglasco located at Unit Nos. 2, 3


and 4, GF LCI Building III, Km. 8, C.P. Garcia Highway (Diversion Road)
Cabantian, Davao City has yet to hire employees of its own;

3. I attest that the Davao Diversion branch has no employees of its own and the
authorized representative assigned to process its registration is an employee of
Davao Cabaguio branch.

4. I execute this affidavit to attest the truth of the foregoing statements and for
whatever legal purpose it may serve.
