FAMILY STRUCTURE ASSESSMENT:

The family structure of the given scenario is primarily categorized as a nuclear family, also known as the primary or elementary family.
The family is composed of a mother which Mrs. Jocelyn Concepcion and her husband as the father of the family. They also have their dependent
children which are Sean, Patrick, Philip, and Ella. In accordance with the descent of the family, it was not mentioned or described in the given
scenario if it was identified as patrilineal, matrilineal, or bilateral. In accordance with the authority, it was mentioned that the breadwinner of
the family or who provides the money is the husband while Mrs. Jocelyn is in charge of budgeting the family expenses and does the household
chores. Lastly, in accordance with the place of residence, the family is neolocal. The family does not reside with the groom or bride’s parents
but they reside independently of their parents which means they can decide on their own as far as their residence is concerned.

GENOGRAM:
1. Frequent drinking of alcohol

Frequent Drinking of Alcohol is a health threat since its long-term health consequences can lead to the development of chronic diseases
and other major issues such as high blood pressure, heart disease, stroke, liver disease, and digestive issues over time. Breast, mouth, throat,
esophagus, voice box, liver, colon, and rectum cancers are all prominent. Also, this health problem was assessed since the father was said to have
a vice which is frequent drinking of alcoholic beverages which led to their debts in the store.

The second level assessment is the inability to make decisions with respect to taking appropriate health action due to failure to
comprehend the nature, magnitude/scope of the problem since the denial about its existence or severity as a result of fear of consequences of
diagnosis of problem, specifically health consequences.

Objectives of Care: Within 4 hours of nursing interventions, the family will be able to:
1. Have enough knowledge about the health risk of frequent drinking of alcohol
2. Develop and Improve Strategies to Prevent Alcohol Misuse
3. Recognize that frequent drinking of alcohol may lead to alcoholism

Criteria Computation Actual score Justification

1. Nature of the 2/3 x 1 2/3 The nature of the problem is given 2 points as a health
Problem threat. The highest possible score is 3 and the weight
of the criteria is 1. Therefore, the computation is 2/3
(1) and the actual score is 2/3.

2. Modifiability of 2/2 x 2 2 The modifiability of the problem is given 2 points

the Problem since it is easily modifiable. Health education
provided by the nurse and support groups for
people with the same problem can help in
modifying this. The actual score of 2 was
computed since the problem has 2 points, the
highest possible score is 2, and the weight of the
criteria is 2.
3. Preventive 3/3 x 1 1 The preventive potential of this problem is given
Potential 3 points since it is high. Alcoholism, debt growth
in the store, and chronic diseases and other
serious problems including: high blood pressure,
heart disease, stroke, and specifically liver
disease will be prevented if this problem will be
addressed. The actual score of 1 was computed
since the problem has 3 points, the highest
possible score is 3, and the weight of the criteria
is 1.
4. Salience of the 0/2 x 1 0 The salience of the problem is given 0 point since
problem the family does not perceive this as a problem or
condition needing change. This is evident by no
verbalization of concern coming from the family.
The actual score of 0 was computed since the
problem has 0 point, the highest possible score is
2, and the weight of the criteria is 1.
Total score 3.67

2. Family size beyond what family resources can adequately provide

Family size beyond what family family can adequately provide is a health threat since earning less than what the family needs can be the
root of many problems some of which are malnutrition, late detection of health deficits of present, and insufficient care for the sick member/s of
the family. Also, in accordance with the typology of nursing problem, this health problem is classified under health threat. Also, this health
problem was assessed since the father’s monthly income of P8,000 for 6 members of the family is not enough for their needs.

The second level of assessment is the inability to provide a home environment which is conducive to health maintenance and personal
development due to inadequate family resources specifically financial constraints or limited financial resources and lack of or inadequate
knowledge of preventive measures.

The family has inadequate resources to sustain the needs of each member. While lack of knowledge is evidence by the number of children, and
their age gap is too short for the 1st 3 children. The family were not able to take preventive measures conducive to appropriate family planning.

Objectives of Care: Within 4 hours of nursing interventions, the family will be able to:
1. Recognize the benefit of creating multiple income streams.
2. Be knowledgeable on how to analyze their financial situation.
3. Accept the importance of proper planning for their family's future.

Financial support
from the government such as 4Ps and Social
Amelioration Program can aid in the family’s
financial resources. The actual score of 2 was
computed since the problem has 2 points, the
highest possible score is 2, and the weight of the
criteria is 2.
3. Preventive 3/3 x 1 1 The preventive potential of this problem is given
Potential 3 points since it is high. Malnutrition, late
detection of health deficits if present, and
insufficient care for the sick member/s can be
avoided if this health problem is acted upon. The
actual score of 1 was computed since the
problem has 3 points, the highest possible score
is 3, and the weight of the criteria is 1.
4. Salience of the 1/2 x 1 1/2 The salience of the problem is given 1 point since
problem the family recognizes the condition or problem
not needing immediate attention. The actual
score of 1/2 was computed since the problem
has 1 point, the highest possible score is 2, and
the weight of the criteria is 1.
Total score 4.17

3. Mild stroke

Mild stroke is a health deficit since the disability is already present. They mentioned in the case scenario that Mrs. Jocelyn suddenly
collapsed and it took long before she was taken to the hospital that turned out to be a mild stroke.

Inability to provide adequate nursing care to the sick, disabled, dependent or vulnerable/ at risk member of the family due to inadequate
family resources for care, specifically financial constraints. This is because the family have no money to pay for the hospital bills of Ms. Jocelyn.

Objectives of Care: Within 4 hours of nursing interventions, the family will be able to:
1. Maintain a stable or improved level of consciousness
2. Have enough knowledge about the expected outcome of the stroke and avoid doing things he or she can do.
3. Be knowledgeable on stress management techniques and maintenance of personal health for family coping.

Criteria Computation Actual score Justification

1. Nature of 3/3 x 1 1 The nature of the problem is given 3 points as a health
the Problem deficit. The highest possible score is 3 and the weight of the
criteria is 1. Therefore, the computation is 3/3 (1) and the
actual score is 1.

2. Modifiabilit 1/2 x 2 1 The modifiability of the problem is given 1 point since

y of the the problem can be partially/moderately modified. The
Problem nurse can use his/her knowledge, skills, and time in
providing appropriate care for Mrs. Jocelyn. However,
the family has difficulty of providing themselves
adequate resources for management of the health
deficit. This is evident by the verbalization of fear of
the family that they have no money to pay for the
hospital bills which led in the early discharge of
the patient. The actual score of 1 was computed since
the problem has 1 point, the highest possible score is 2,
and the weight of the criteria is 1.
3. Preventive 3/3 x 1 1 The preventive potential of this problem is given 3
Potential points since it is high. Paralysis, difficulty swallowing
or talking, balance problems, and dizziness, the
complications of mild stroke if left untreated, can be
avoided if the problem is addressed and proper health
care is provided. The actual score of 1 was computed
since the problem has 3 points, the highest possible
score is 3, and the weight of the criteria is 1.
4. Salience of 2/2 x 1 2 The salience of the problem is given 2 points since the
the problem family recognizes the condition or problem not
needing immediate attention. This is evident by
family’s action to prioritize the avoidance of debt or
the financial problem than the hospitalization of Mrs.
Jocelyn. The actual score of 2 was computed since the
problem has 1 point, the highest possible score is 2,
and the weight of the criteria is 1.
