That I was issued a Philippine Passport by the Department of Foreign Affairs Office in
Legazpi City, Albay;

That last 27 July 2013 at around 4 o’clock in the afternoon, I parked my car in front of
Tableu Grill along Leon Aureus St., Magsaysay Ave., Concepcion Pequeña, Naga City;

That after a while, I went back to my car and found that the front passenger door was
already open and slightly ajar;

That after noticing it, I immediately look for my black bag which I left inside my car that
contains my Passport, SSS ID, TIN ID, Bank Passbook, cash, among others;

That the above-mentioned black bag of mine was nowhere to be found inside my car
along with its contents;

That I tried looking for it and despite diligent effort to locate it the same prove futile;

That as a consequence of such, I endeavoured to have the said incident reported and have
it recorded before the Police Station 2 of Concepcion Pequeña, Naga City;

That I am executing this affidavit to attest to the truth of the foregoing statements and for
the purpose requesting for a reissuance of my Philippine Passport before the DFA of Legazpi City,
Albay, and for whatever legal purpose this may serve.

That I am executing this affidavit to attest to the truth of the foregoing statements and for
the purpose of requesting for the reissuance of the OR and CR of the abovementioned motor
vehicle from the Land Transportation Office (LTO), and for whatever legal purpose this may serve.

procured by the Armed Forces of the Philippines, 22nd Infantry Battalion Philippine Army,
figured in a vehicular accident, by hitting the person of one Roly F. Suarez;

3. That while the aforementioned Truck was traveling the road of Zone 1, Brgy. Caroyroyan,
Pili, Camarines Sur, in official business, carrying supplies that were to be delivered to
different Army Detachments of the Philippine Army, was being carefully and slowly
driven, prompted PFC Atian to step on the brake when a male person, later identified as
Roly F. Suarez, the victim herein, was accidentally bumped which caused injury and death
to Roly F. Suarez;

4. That the accident occurred when the victim herein suddenly crossed the road;

5. That as the Headquarters Commandant, I would like to take possession of the detailed
Truck since it is being used for official businesses of the Philippine Army, 22nd Infantry
Battalion;

6. That I hereby undertake to present the said motor vehicle upon request of the police or
any judicial tribunal if needed for evidence purposes;

7. That I am executing this Affidavit of Undertaking for the release of the said Motor Vehicle
(Truck) from the Pili Police Station and for whatever legal purposes this may serve.
