observed at all times)

3- Satisfactory (Carries out procedures efficiently and systematically but requires minimal guidance and
supervision/ Personality trait is observed most of the time)

2- Fair (Carries out the procedures efficiently and systematically but requires close guidance and supervision/
Personality trait is seldom observed)

1– Poor (Carries out the procedures inefficiently, unsystematically even under close guidance and
supervision/ Personality trait is never observed)

I. Safe and Quality Nursing Care
1. Conducts an accurate, organized/systematic and complete assessment using
appropriate techniques (i.e. auscultation, inspection, screening tests, interpretation
of lab data etc.)
2. Performs comprehensive organized obstetric health history
3. Identifies normal vs. abnormal findings on health assessment
4. Effectively establishes priorities (both actual & potential)
5. States nursing diagnosis accepted by NANDA
6. Formulates a plan of care in collaboration with clients and other members of the
health care team
7. Provides holistic care that enhances quality of life, physical needs, psychological
needs and spiritual needs
8. Utilizes theoretical knowledge (e.g. physiology, pharmacology, evidenced- based
practice, principles of primary health care) as a basis for nursing interventions of
clients at all levels of the health-illness continuum.
9. Implements plan of care according to the client’s needs and established priorities.
10. Follows prescribed medical regimen for the client undergoing diagnostic tests and
therapeutic procedures.
II. Management of Resources and Environment
1. Organizes work load to facilitate timely client care.
2. Adheres to policies, procedures, and protocols of the institution
3. Demonstrates an ability to adapt the plan of care in changing situations (e.g. new
assignment, changes in condition.)
III. Health Education
1. Assesses individuals’ and families’ leaning needs (e.g. medical treatment, nursing
interventions, lifestyle changes, health promotion)
2. Develops, implements and evaluates appropriate teaching strategies.
3. Provides education regarding current health problems, health promotion, and
disease prevention to client’s relatives.
IV. Legal Responsibility
1. Adheres to legal and institutional protocols regarding client’s rights.
2. Maintains confidentiality and privacy of client’s data.
3. Confirms information given by the doctor for informed consent
V. Ethico - Moral Responsibility
1. Practices within the ethical and legal framework of nursing
2. Accepts accountability and responsibility for own decisions and actions
3. Respects the rights of clients
VI. Personal and Professional Development
1. Establishes and maintains a positive working relationship with Clinical Instructor,
clinical group, and other members of the health care team
2. Possesses positive attitude towards change and criticism.
3. Projects a professional image of a nurse.
VII. Quality Improvement
1. Identifies significant changes in client’s condition to improve stay in the hospital.
2. Participates actively during endorsement and nursing rounds.
3. Recommends solutions to identified problems.
VIII. Research
1. Identifies appropriate methods of research for a particular client
2. Utilizes a variety of resources to incorporate evidence-based practice into plan of
care and/ or other assignments.
3. Disseminates results of ER related research findings to clinical group and other
members of the ER team as appropriate.

IX. Records Management


1. Documentation is thorough, accurate, concise, updated in a timely manner
2. Maintains integrity, safety, access, and security of records.
3. Utilizes hospital records system such as Kardex, Chart, etc.

X. Communication
1. Establishes and maintains effective communication with clients, families, significant
others and health team members.
2. Communicates effectively and appropriately with members of the health care team
to provide continuity and comprehensiveness of care.
3. Establishes a climate conducive to communication.
XI. Collaboration and Teamwork
1. Collaborates plan of care to the Clinical Instructor and other members of the health
care team.
2. Demonstrates willingness to work as a team member and assist others.
3. Establishes collaborative relationship with group members, Clinical Instructor, and
other members of the health care team.
