happened. An incident report should
be completed by the injured employee
to offer a summary of the events that
led to the accident. All witnesses with
firsthand knowledge of the accident
should be interviewed, and each
witness should complete a witness
statement.

Step 4: Employers should photograph


the accident scene, including any
objects, damp floors, or equipment
that may have contributed to the
accident.

Step 5: Companies should call their


insurance companies and file a report
of a work-related injury. Employers
should give all information necessary
by the insurance carrier, as well as
statements and any other applicable
supporting documentation.
Step 6: After the accident
investigation is completed, follow up
with the employee to see how he or
she is doing. If the employee first
refused medical treatment, the
employer should check with the
employee that no medical treatment
was required as a result of the injury.

Step 7: Corrective action should be


determined in order to prevent a
recurrence and improve the
company's overall operations. If an
employee breaks a major or minor
safety rule, the employer must follow
the company's work rule violation
policy and take the appropriate
corrective action.
