Ref: MTCPL/SWSM/TPI-Cluster 17/2022-23/4435 Date: 30-05-2022

During a site visit in Varanasi District, the Third Party Inspection Agency (TPIA) observed several non-conformities in the construction work being executed by the contractor Larsen and Toubro Ltd. for the Varanasi Tubewell Scheme. The non-conformities included cracks in precast elements of an overhead tank, substandard cement concrete and brick masonry work, incorrect rebar placement, and missing approval documents. The contractor was advised to rectify the issues and submit compliance to the TPIA.

observed in Varanasi under Varanasi Tubewell Scheme - Reg.

Sir,
It is to inform that during site visit in Varanasi District on 23-05-2022 where, Overhead Tank / Elevated
Storage Reservoir ( OHT / ESR ), Pump House, Boundary Wall, Solar based LED, Tubewell, Installation of
Pumping Plant work is being executed by Contractor , M/s Larsen and Toubro Ltd. for Varanasi Tubewell
Scheme, some non-conformities were observed by TPIA. Detail of non-conformities/discrepancies observed is
tabulated below.

Other Observations:
NA
As per TKC RiTika Aditya green energy solar panel will be installed at site but no approval letter for same is
available with TKC.
 
The site representative of the Contractor has been advised by us to attend to the non- conformities and
improve/implement the safety culture at site from ground level. The photographs of the NCR are attached
here with for your ready reference. The Contractor may kindly be instructed from your end also to rectify the
non-conformities at the earliest and submit the compliance with a copy marked to TPIA as well before
proceeding to next stage of activity.
