Additive To Additive To Hyperbaric Bupivacaine in Spinal Anesthesia

This research proposal compares the effects of dexmedetomidine and clonidine as additives to hyperbaric bupivacaine in spinal anesthesia. The study aims to compare the onset and duration of sensory and motor block, as well as hemodynamic changes and sedation levels. It is a double-blinded randomized study of 30 patients undergoing spinal anesthesia who will receive either clonidine or dexmedetomidine added to bupivacaine. Data on blood pressure, pulse rate, oxygen saturation and urine output will be collected to analyze the effects. The researcher hypothesizes that both additives will shorten the onset and duration of sensory and motor block compared to bupivacaine alone.

RESEARCH QUESTION:

HOW DOES INTRAVENOUS DEXMEDITOMIDINE AND


INTRAVENOUS CLONIDINE PROLONG THE EFFECT OF
BUPIVACAINE IN SPINAL ANESTHESIA ?

BACKGROUND:

The purpose of this study is to compare the onset and duration of sensory and
motor block , as well as hemodynamic changes and level of duration of sensory
and motor block, as well as hemodynamic changes and level of sedation
following intrathecal bupivacaine supplemental with either dexmeditomidine or
clonidine.

RATIONALE OF THE STUDY IN THE LIGHT OF A THOROUGH


LITERATURE REVIEW:

Dexmeditomidine is alpha 2- agonist used as study of dexmedetomidine


and clonidine as additive to hyperbaric bupivacaine in spinal anaesthesia

 clonidine is a partial agonist at alpha 2-adrenoreceptors


 bupivacaine is a local amide anesthetic
 clonidine in combination with local anesthetic prolongs
analgesia and motor blockade

 dexmeditimidine with local anaesthetic increases the quality


of subarachnoid block

LACUNAE OF THE STUDY:

 We lack to follow up the patient .


 Lesser number of patients.

NOVELTY OF THE STUDY:

Many studies have shown that clonidine versus dexmeditomidine


in additive to hyperbaric bupivacaine. in this study, it is mainly focused on the
duration and onset of action of clonidine and dexmeditomidine in spinal
anaesthesia

 HYPOTHESIS:
To compare dexmeditomidine and clonidine as additive to hyperbaric
bupivacaine

AIM AND OBJECTIVES OF THE STUDY:

To compare the effect of addition of equipotent doses of clonidine


0.5ml and dexmeditomidine 100mcg in 0.5 normal saline added to 2.5ml of
0.5% hyperbaric bupivacaine in spinal anaesthesia

METHADOLOGY:

Double blinded randomised study

INCLUSION CRITERIA:
 Patients in age group of 18 to 70 yrs
 ASA I,II,, III

 EXCLUSION CRITERIA:

 patient refusal
 allergic to drugs
 renal disfunction

SAMPLE SIZE

30 patients

DATA COLLECTION:
 blood pressure
 pulse rate
 mean arterial pressure
 oxygen saturation
 urine output

EXPECTED OUTCOME:
I expect as addition of clonidine and dexmeditomidine additive to
bupivacaine in subarachnoid block,shortens the onset and duration of both
sensory and motor block.

SCOPE OF STUDY :
In patients undergoing spinal anesthesia ,dexmeditomidine can
be used as alternative drug in subarachnoid block because of easy
availability than clonidine
