I. Purpose
This policy provides direction for the processes and procedures to ensure availability of
laboratory results during equipment malfunction.

II. Scope
This policy covers all laboratory machines and equipment whether tie-up or purchased by St.
Andrew’s Hospital.

III. Responsibilities
a. The Chief Medical Technologist shall review requests for the repair, transfer, disposal, or
trade-in of equipment no longer required for use. She/he shall also manage the operation,
maintenance and calibration of laboratory equipment, coordinate corrective and preventative
maintenance programs. She/he shall retain records of selection, evaluation, validation,
maintenance, and calibration

b. Laboratory testing personnel shall refer to operator manual for troubleshooting and must
always inform the Chief Medical Technologist regarding problems in laboratory equipment.
She/he shall also operate, maintain, and calibrate laboratory equipment and must record and
report corrective and preventative maintenance

IV. Definition of Terms

Troubleshoot - the process of diagnosing the source of a problem. It is used to fix problems
with hardware, software, and many other products. Its basic theory is that you start with the most
general (and often most obvious) possible problems, and then narrow it down to more specific
issues.

V. Policy

a. The laboratory makes appropriate use of, maintains and services all equipment and supplies.

b. Whenever defective machine cannot be repaired immediately, refer to procedure on


outsourcing of laboratory examinations.

VI. Procedure

LABORATORY EQUIPMENT MALFUNCTION

a. It is appropriate for the laboratory staff to first identify the nature of the equipment
breakdown. Conferring with the equipment operator manual can help the laboratory
staff to identify such problems. Note the error codes displayed on monitors to easily
assess the problem.

b. Notify the Chief Medical Technologist and the responsible technician as soon as
possible.

c. Equipment back-ups must be available for all equipment performing routine


procedures such as microscopy, hematology analyzer and chemistry analyzer.

d. In cases where no machine back-up is available, refer to policy on referral and


outsourcing of laboratory examinations to avoid delay in release of results.

e. Record the findings on possible defects in the machine in the Equipment Maintenance
Log Sheet.

f. Compile the preventive maintenance records in order to assess the performance of


the machine.
