INFORMATION

The undersigned Assistant City Prosecutor of the City of Cebu hereby


accuses for Violation of Sec. 3(c) of R.A.
9287, committed as follows:

That on ______________ , at more or _________ P.M.,


in the City of _____, Philippines, and within the jurisdiction
of this Honorable Court, the said accused, being then in pos-
session of gambling paraphernalia consisting of the follow-
ing:

1) one (1) stub paper used as tally sheet with different


bet number combinations written;
2) one (1) ballpen; and
3) Bet money amounting to Php ______ in different de-
nominations

with deliberate intent, and without being authorized by the


Philippine Charity Sweepstakes Office (PCSO), did then and
there collect, solicit and/or produce bets from the public in
behalf of his principal or financier for an illegal numbers
game locally known as “masiao” the result of which is de-
rived from the Swertres game result of the Philippine Charity
Sweepstakes Office (PCSO) by issuing number combinations
as factors in giving out jackpot to an unknown bettor who
was able to elude arrest but will be prosecuted as soon as
identified and procedural requirements are complied with.
