Affidavit of Designating A Legal Beneficiary

JR I. Palafox designates his mother, Warlina Palafox, as his sole beneficiary for any benefits from the Armed Forces of the Philippines in the event of his death or injury. The designation only applies to AFP benefits and does not affect his other properties. If he survives his mother, any AFP benefits will be included in his estate. AFFIDAVIT OF DESIGNATING A LEGAL BENEFICIARY

I, JR I. PALAFOX, of legal age, single, and presently residing at No. 45B Road 7 Project
6, Quezon City, duly sworn in accordance with law, depose and state that:

1. I do hereby intend and declare that upon my death or any


untoward incident, I am willingly designating my mother, WARLINA PALAFOX
as my sole beneficiary;

2. I understand that the designated beneficiary will be listed on the


record of the Armed Forces of the Philippines in relation to my application as a
Cadet Soldier;

3. I understand that the designation of a beneficiary has no effect on


my other properties asides from that which will be granted to me by the Armed
Forces of the Philippines as a Cadet Soldier and to those that I may cancel or
change the designation at any time without the requirement of consent of the
aforementioned beneficiary;

4. I further understand if I survive the designated beneficiary the


items which will be given by the Armed Forces of the Philippines must be
included in my probate estate.

5. I also understand that any transfer to my designated beneficiary as a


result of this affidavit is not testamentary.
