SUBJECT: Fire Prevention and Firefighting Plan

TO : All concerned workers of the plant.

I. INTRODUCTION:

Dry season is now approaching in the entire country and the awareness of the fire prevention is in
progress, as the fire prevention month all over the country now being observed in this month of March;
incidents or reports emanating from Fire/Conflagration it increasing during the observance of fire
prevention month.

II. SITUATION:

Greenergie plant occupying the more or less 7.5 hectares of land area and same was located along
the river of bank in Brgy Longos, Pulilan, Bulacan. The plant has erected/established eight (8) buildings, (1) for
Main Office/Admin Building, one (1) for storage/bodega, three (3) buildings for scrap, garbage and with stock
room, newly built motor pool and building which is use for water waste treatment located at north east
portion of the plant.
In cases fires/conflagration emanates from unknown source happened at the north portion of the
main office building, simultaneous execution of all workers to prevent the proliferation/escalation of fire in
said area, workers shall be organized into the following: Fire Fighting Team, Fire Rescue and Evacuation
Team, Medical Team and the Fire Marshal.

III. SCENARIO:

When fire/conflagration broke up at the north west portion of the administrative building were the
main office is located, the immediate deployment of fire track tank shall be immediately position near the fire
scene area with responder simultaneous movement rescue/evacuation, medical teams and the fire Marshalls
shall move and occupy their respective place or area of deployment and immediately perform their
respective tasked. Following are the tasks of the different teams:

IV. TASKING:

a. FIRE FIGHTING TEAM ”A” & “B” – shall be act as Ist Responder and move by passing thru the
Building #1 with fire fighting materials proceeding towards the fire scene area and also responsible to assist
the fire track tanker to focus the hose of same to the center of fire to prevent from escalating same. While
Fire Fighting Team “B” shall move towards the fire scene by passing thru the second gate then at the back of
Pyroclave Building leading to the fire scene.
Perform other tasked as directed.

b. FIRE RESCUE AND EVACUATION TEAM (“A” & “B”) – Team “A” shall be responsible to rescue
casualty of fire in the fire scene and to bring same to the safer area or to the established medical tent and
also Team “B” shall assist the office personnel in taking out from the office all important documents that may
caught by fire and bring same to the safer area; and
Perform other tasked as directed.

c. MEDICAL TEAM – shall act and execute in rendering first aid to the casualties, afterwhich
same shall be brought to the nearest hospital for medical attention; and
Perform other tasked as directed; and

d. FIRE MARSHAL TEAM – shall responsible to prevent the by passers in coming nearer to the
fire scene area to prevent them in accidentally being caught by strong heat of fire and to secure the area
from looters and prevent to cause damage and injuries; and
Perform other tasked as directed.
V. EXECUTION:

Upon receipt the of report re-fire happened in said area/place, all mentioned teams shall
immediately apply and execute the following: FIRE FIGHTING TEAM (“A” & “B”) shall act as the first
responder, with fire extinguisher and other fire fighting materials to proceed at the fire scene to help for the
control and proliferation of fire and also assist the fire track tanker full of water in focusing the water hose
directly to the area/center of fire to avoid same to spread/scattered that will result same to control. Likewise
the RESCUE AND EVACUATION TEAMS “A” shall immediately respond to the reported fire incident and
established Command Post at the adjacent vicinity of fire ready to take effect the rescue operation on the
reported casualty in the fire scene. While Team “B” shall proceed to the office and assist the admin
personnel to evacuate some vital document of the Office and to bring same to the safer area; and the
medical team shall also ready on call to impart some medical assistance thru first aid to the reported casualty
who incurred/suffered third degree burn in the body brought about by said fire.

visit/inspection of NESTLE TEAM
1. At exactly 8:00 o’clock in the morning
of 21 January 2016 all workers must be in their respective
place of work and must see to it that items under their
works are properly arrange and not disarray and those sharp
object which may prone from accident must properly
secured;

2. Two (2) hours before the arrival the inspection

team, Tanker loaded with water shall be responsible to
spree water along the route from the main gate to 1, 2 and 3
buildings to ensure that the visitors will not suphocated
from the thick of dust during their inspectiom;

3. Track drivers shall ensure that their respective

driven vehicle was properly car washed and presentable and
must be parked at the designated parking area infront of the
admin building;

4. No slippers, short pants and sando during the day

day inspection, workers must be in complete company
uniform with Identification Card with pictures pinned at
their respective breast;

5. Strictly “NO SMOKING” during the inspection;

6. Duty Gate Guard to refrain all buyers and visitors to

come in during the inspection; and

7. Hundred percent attendance of all workers is

required.

sumailalim sa pagiinspeksyon ng guadiya sa kanilang dala dalang bag at body fisking sa main gate bago sa
malayang makalabas sa kanilang pag uwi;
b. Ganun din sa mga driver na dala ang kanilang menamanihuhang sasakyang palabas ay
nararapat lang na dumaan o sumailalim din sa inspeksyon at kung ang nasabing driver na kasama ang
menamanihuhang sasakayan ay kailangang may dala itong GATE PASS bilang patunay na ang kanyang lakad
ay awtorisado at legal;
d. Sa panahon ng kanilang pagganap ng tungkulin ng guardiya lalo na sa gabi ay may
ilang tauhan ng kompanya na pumapasok pa rin na kanilang rason ay may gagawin na ayon dito ay
pinapagawa sa knaila sa loob, kung kaya’t para maiwasan ang hindi magandang mangyayari ay hinihiling din
ng guardiya na mabigyan sila ng kopya ng mga pangalan na naatsang mag overtime o kung hindi naman ay
mismo ang taong naatsan a/ hawak papel bilang patunay na ito ay legal na may permiso upang gumanap sa
isang Gawain sa oras ng gabi; at

oras ng madaling araw, kung kaya para lalong mapaigi at maging mapaayos ang pagtupad ng kanilang

2. In reference above, please be informed that on or about 9:00 o’clock in the afternoon of 14
March, 2016 the undersigned representative of this corporation attended the scheduled seminar/briefing
Fire Prevention Awareness in consonance with the observance of FIRE PREVENTION MONTH held at the
conference room of NESTLE CORPORATION in Brgy Tibag, Pulilan, Bulacan and afterlunch at about 1:00
o’clock in the afternoon fire drill exercises was followed and the activity was participated in by all employee
headed by their respective Department Head, Safety Officers of the different Construction Firm contracted in
said corporation supervised/headed by MR ,MIKE CORPUZ, Safety Officer, Head of Nestle Philippines Inc. in
Pulilan. Said activity was made in coordination with Pulilan Fire Station headed by SFO3 Doncil, Chief Clerk of
Pulilan Fire Station which was imparted on the subject of Fire Prevention aspect, while SFO3 MARIO
BERNADDO spearheaded the supervision of the different tasked group on their movem ent to take effect their
respective tasked to prevent the escalation fire. Further, said fire officer demonstrated to the attendee the
use/operation of fire extinguisher, where the three (3): ROMEO SORRO, JOSEPH ABULENCIA and JEFFREY
ESGUERRA all members of GREENERGIE CORPORATION was able to try the proper handling and operational
aspect of fire extinguisher to distinguished fire.

2. Activity ended at about 3:30 same date with fruitful and successful result.

1. Reference: Observance and Preparation re-Fire Prevention.

2. In reference above, please be informed that on or about 2:00 o’clock in the afternoon of 22
January 2016 at about 2:00, Pulilan BFP Team composed of eight (8) personnel headed by SFO3 Doncil, Chief
Clerk of Pulilan Fire Station lectured on the subject of Fire Prevention aspect, while SFO3 MARIO BERNADDO
spearheaded the supervision of the different tasked group on their movement to take effect their respective
tasked to prevent the escalation fire. Said fire drill was participated in by all personnel/workers of this
Corporation under the overall supervision of MS PATRICIA ANNE P CASTLLIO, Plant Manager. FO2 POL CADIZ,
Operation Officer of said Fire Station demonstrated the proper handling and operational aspect of fire
extinguisher to distinguished fire, after the demonstration workers, one by one try to operate on how to
neutralize fire. While the Rescue Team was able to execute the rescue operation which resulted to extract
from the scene of fire the reported victim and same was brought to the established medical team to impart
some first aid to make same stable and rushed the victim to the nearest hospital for further medical attention
needed.

2. Activity ended at about 4:30 same date with fruitful and successful result.

Act Bilang 5487 (Security Agency Law) na ang PAGSUOT NG TAMANG UNIPORME SA TUWING
NAKA DUTY;
2. Walang ginawa na tamang relasyon sa kapuwa niya guardiya, bagkus ito pa ang
pasimuno ng hindi pagkaintindihan sa bawat isa na siyang sanhi ng pagkakawatak watak sa bawat
isa at hindi niya nagawang pag isahin ang mga ito tungo sa pagtupad, pagtalima at pagsunod para
ipalaganap ang iisang layunin ng kompanya na katiwasayan at kapayapaan dito, lalo’t higit ang
kawalan niya ng tiwala , respeto pagkilala sa SAFETY OFFICERS dito;
3. Malimit pa itong gumawa ng mga ulat sa pamamagitan ng TEXT MESSAGES lang at
