NAFDAC REQUIREMENTS FOR FACTORY SET UP & INSPECTION.

DOCUMENTS TO BE SUBMITTED FOR LICENCE INSPECTION.

APPLICANTS FOR NEW LICENCE OF THEIR FOOD PRODUCTS SHALL SUBMIT:

THREE (3) COPIES OF EACH OF THE FOLLOWING DOCUMENTATS:

1. Application for production Inspection on company Letter - head; addressed


to the "Director General NAFDAC, ATTENTION : Your State coordinator"
indicating product(s) for registration and factory address.
2. Receipt of payment issued by NAFDAC.
3. Certificate of Incorporation of Company from Corporate Affairs
Commission.
4. Evidence of Trademark Registration ( Acknowledgement & Acceptance) in
the relevant class.
5. Label/Package with appropriate storage conditions indicated
6. Appropriately filled NAFDAC Registration form.
7. Certificate of Analysis for Raw material and finished products (The Mfg
Date, BB Date, and Batch Number of the Finished Product must be stated on
the Certificate of Analysis of the Finished and Both Certificate must be
stamped and endorsed by the PUBLIC ANALYST).

AND

ONE (1) COPY EACH OF THE FOLLOWING DOCUMENTS:

8. Food Handlers' Test Certificate or Medical Certificate of Fitness.


9. Agreement with fumigation company.
10. Current Certificate of Fumigation.
11. List of production and Quality Control Equipment ( where applicable) with
their sources.
12. Batch Formulation for products with more than one ingredient.
13. Geological survey ( minimum depth : 150ft or 45.72m) and Borehole
completion report having all pages of the report stamped and signed by
borehole company's CEO.
14. Production Flow Chart for each product.
15. Organization chart for the company showing names of key personnel.
16. Letter of Employment of key officers and Acceptance of offer of
employment by these officers.

18. Coloured passport photographs of both Production Manager and Quality
Control Manager.
19. Telephone Numbers and Email addresses of the MD and other key
officers of the company.
20. Route Map From a well-known location to the factory, showing key
landmarks.
21. Standard Operating Procedures (SOP) for Production
22. Standard Operating Procedures (SOP) for Quality Control.
23. Standard Operating Procedures (SOP) for Cleaning of Factory and
Equipment.
24. Standard Operating Procedures (SOP) for Maintenance .
25. Standard Operating Procedures (SOP) for Recall and Release of products.
26. Contract Manufacture Agreement (where applicable)
27. Notification of product registration (full or Listing).
