Centrifugal Pump Commissioning Report
The following checklist should be used by the installation technician as a record of initial start-up conditions.

Motor HP & Speed V/Hz/Ph Impeller Diameter Motor Serial No. Service OK Fail 1 Insure that the Operating Manual is available for reference 2 Turn off and Lockout/Tagout the power supply 3 Inspect the equipment for any damage or missing parts/components 4 Insure the equipment is resting on all four corners and is level in both directions 5 Secure the equipment to the mounting surface to avoid movement during operation 6 Insure that the areas surrounding the equipment have at least 24" clearance 7 Check the mounting and flange bolts have been tightened 8 Insure suction and discharge piping is anchored independently of the pump 9 Insure suction line is equal or larger than the pump suction size 10 Insure a shut off valve is installed in the suction line 11 Insure suction line has a straight section into the pump at least 2 times its diameter 12 Insure a throttle valve is installed in the discharge line 13 Insure that pump rotates freely by hand 14 Insure coupling is aligned 15 Insure suction valve is fully opened 16 Insure discharge valve is slightly open, not fully open. 17 Discharge valve fully open valve after pump is running 18 Verify the voltage setting on the motors 19 Connect the incoming power to the starter as per the voltage required 20 Check motor rotation (counter-clockwise when viewed from the suction side) 21 No lubrication is required on the bearings until after the first year of operation
