Alabang, Muntinlupa City, having been duly sworn to in accordance with law, do hereby depose
and say that:

1. I am the Corporate Secretary of COMPANY NAME. (the “Corporation”), a corporation duly


organized and existing under the laws of the Republic of the Philippines and with office
address at Joseph Street, San Jose Village, Alabang, Muntinlupa City.

2. At a special meeting of the Board of Directors held on the 13th day of October 2015, the
Board has unanimously approved.

“Resolved, as it is hereby resolved that the corporation shall open and maintain an account
with RFID and the following below vehicles are authorize to enroll under the company’s RFID
Account.”
