LAST WILL AND TESTAMENT

OF PATRICK WILLIAM SHORT
 
KNOW ALL MEN BY THESE PRESENTS:

I, PATRICK WILLIAM SHORT, of legal age, Canadian Citizen, born on the 20th of
December, 1949, single with minor child (Gwyneth Cornelio Short), and currently residing in
Royal Palms, Dumaguete City, Province of Negros Oriental, Philippines, being of sound and
disposing mind and memory, and not acting under undue influence or intimidation from anyone,
do hereby declare and proclaim this instrument to be my Last Will and Testament, in English, the
language which I am well conversant. And I hereby declare, in accordance with my national
law, that:

1) If I should die, I wish for my body to be embalmed and at St. Peter’s Chapel located in
Banilad, Bacong, Negros Oriental and held until the arrival of Margaret A. Cornelio,
with residence 1325, Lucerne Street, Helvetia Heights, Bacolod, Negros Occidental, in
order that she can facilitate the vigil and viewing of my body and thereafter cremation of
my body. The person I designate to be in charge of facilitating this is Manelyn C.
Azucenas residing in Bacolod City, Negros Occidental, who shall use my personal funds,
for funeral expenses only, from my personal Savings Account under Rizal Commercial
Banking Corporation (RCBC), Dumaguete Branch, under the name Patrick W. Short ITF
Gwyneth C. Short, with Savings Account No. 0000001428406296, currently under
Passbook Serial No. 3749558, including the possession and use of the visa and debit bank
cards attached to the above mentioned account with Nos. 4311 7006 8030 7015 and 4293
8215 4092 4005, respectively;

2) I own the following properties situated in the Republic of the Philippines and desire them
to be disposed of in the following manner upon my death:

a. It is my wish and desire to bequeath, grant and devise all my personal properties
and belongings in my current residence at Royal Palms, Dumaguete City, Negros
Oriental, or wherever my residence may be at the time of my death, to my loving
daughter Gwyneth Cornelio Short;

b. It is my wish and desire to bequeath, grant and devise my dog, Shilo, a Rottweiler,
to my good friend Merle Fondren, residing in Tunga-tunga, Dauin, Negros
Oriental;

c. It is my wish and desire to bequeath, grant and devise any monies in my Rizal
Commercial Banking Corporation (RCBC), Dumaguete Branch, with account
numbers my personal Savings Account under Rizal Commercial Banking
Corporation (RCBC), Dumaguete Branch, under the name Patrick W. Short ITF
Gwyneth C. Short, with Savings Account No. 0000001428406296, currently
under Passbook Serial No. 3749558, including the possession and use of the visa
and debit bank cards attached to the above mentioned account with Nos. 4311
7006 8030 7015 and 4293 8215 4092 4005, respectively to Gwyneth Cornelio
Short, with residence 1325, Lucerne, Helvetia Heights, Bacolod, Negros
Occidental, to be held in trust by her mother, Margaret A. Cornelio, until the
former is of legal age;

3) I own the following properties situated in Canada and desire them to be disposed of in the
following manner upon my death:

a. It is my wish and desire to bequeath, grant and devise all the gold coins, which I
have entrusted my good friend in Canada, Michel Gloanec, residing in Langley,
British Columbia, Canada, to purchase, to my loving daughter Gwyneth Cornelio
Short;

b. It is my wish and desire to bequeath, grant and devise any monies in my personal
bank account under Prospera Credit Union, located at 15288, #100, Fraser
Highway (Fleetwood Branch, Canada), with Account No. 206-26-10, including
the possession and use of the bank card attached to the above mentioned account
with No. 6271 3500 0098 3385 to Gwyneth Cornelio Short to be held in trust by
my good friend in Canada, Michel Gloanec, until the former is of age;

4) Any properties coming into my legal possession, ownership, or title situated in the
Philippines or Canada, which I may have acquired after the execution of this Last Will
and
Testament through inheritance or by any other voluntary means, shall likewise be
bequeathed, granted, and given to my entrusted and beloved daughter Gwyneth Cornelio
Short;

5) I designate Margaret A. Cornelio to initiate probate proceedings for this Last Will and
Testament in the Philippines and, thus, name her as the executrix of my personal
properties and assets in the Philippines and in case of her inability, unwillingness, or
negligence to do the same, I name and designate Gary Fondren, residing in Tunga-
tunga, Dauin, Negros Oriental to do the same, as her substitute;

6) I also designate Michel Gloanec, to be the executor of this Last Will and Testament in
the Canada in order to reprobate this Last Will and Testament concerning any properties
thereat in accordance with my aforementioned wishes in paragraphs (3) and (4);

7) The executors and administrators I hereby nominated shall be excused from posting any
bond;

8) I revoke, set aside and annul any and all of my other will or testamentary dispositions that
I have made, executed, signed or published preceding this Last Will and Testament.

IN WITNESS WHEREOF, I have hereunto affixed my signature this July 1, 2019, in

the Municipality of Bacong, Province of Negros Oriental, Philippines.

ATTESTATION CLAUSE  
We, the undersigned attesting witnesses, with domicile in the Philippines,
do hereby affirm that the forgoing is the Last Will and Testament of PATRICK WILLIAM
SHORT and we certify that the testator executed this document while of sound mind and
memory. That the testator signed this document in our presence, at the bottom of his
testamentary dispositions and on the left hand margin of every page, and we, in turn, at the
testators behest have witnessed and signed under a free and voluntary act and deed in the same
manner as the testator, in the presence of the testator and of the Notary Public, this 1st day of
July, 2019, in the Municipality of Bacong, Province of Negros Oriental, Philippines.

All known to me and to me known to be the same persons who executed the foregoing Last Will and
Testament, the first as testatrix and the last three as his instrumental witnesses, and they respectively
acknowledged to me that the same as their own free act and deed which the testator executed and
signed in the presence of the three (3) above-named attesting witnesses, who all signed their names
as proof of their attestation on the previous page before the Testator PATRICK WILLIAM
SHORT, and in the presence of each and every one of them, and they acknowledged the same to be
their free and voluntary act and deed.
This Last Will and Testament, with the header “LAST WILL AND TESTAMENT OF
PATRICK WILLIAM SHORT,” consists of three (3) pages, including the page on which the
ratification and acknowledgment are written, and has been signed on every page thereof by the
testatrix and his witnesses, and sealed with my notarial seal.
