The researchers are requesting approval from the Dean/Assistant Campus Administrator of NORSU-Bais to conduct an action research study titled "Truancy Among the Grade 7 Students of Manjuyod National High School". They are fulfilling the requirements for their Methods of Research course. They are also requesting permission from the Principal of Manjuyod National High School to interview grade 7 students there as part of collecting data for their study. The researchers assure that participation is voluntary and confidential.

THE GRADE 7 STUDENTS OF MANJUYOD NATIONAL HIGH SCHOOL” This is in
regard as the partial fulfilment of the requirements in Methods of Research (ED 303) subject.

In pursuit of the success of the objectives of our study which is to propose an action plan in
line with our chosen the research title and that we are going to conduct our study at
Manjuyod National High School. We would like to ask permission to your good office to
conduct this study provided that our research adviser signed and approved this letter.

Dear Madam:

Warmest greetings!

In partial fulfilment of the requirements in Methods of Research (ED 303) subject, we, the
third year Bachelor of Secondary Education Major in English students would like to ask
permission to allow us conduct an interview among the students in your school. This is in
view of our action research entitled, “TRUANCY AMONG THE GRADE 7 STUDENTS
OF MANJUYOD NATIONAL HIGH SCHOOL”. We are conducting this survey by
interview only and among the grade 7 students in Manjuyod National High School, Sundo-
an, Manjuyod. Attached herewith is the interview guide questions for this study.

In the process of gathering data, the survey would only last about 10-15 minutes and would
be arranged at a time convenient to the students schedule (e.g. break). Participation in this
study is entirely voluntary and with such permission to the respondents. There are no known
or anticipated risks as to participating in this study as all information provided will be kept in
utmost confidentiality and would be used only for academic purposes.

Your approval to conduct this study will be greatly appreciated. Thank you in advance for
your interest and assistance with this research.
