I, MORRIS GLENN E. NANO, of legal age, married, Filipino, and resident of


141 Talogtog, Laoac, Pangasinan, Philippines, after having been duly sworn in
accordance with law, hereby depose and state:

That I am the biological father of GLENN GIAN P. NANO who is a minor and
a citizen of the Republic of the Philippines and a holder of valid Philippine Passport
with No. _____________ and who is going for a trip abroad specifically to Milan,
Italy on _____________;

That I am giving my full consent to my said son to travel abroad specifically


to Milan, Italy ;

That further, GLENN GIAN will be accompanied or escorted by his mother


LILIAN PAMINTUAN NANO in his travel abroad;

That I am hereby giving my full consent to the said LILIAN PAMINTUAN


NANO to accompany or escort GLENN GIAN in his travel abroad;

That I am executing this affidavit to declare the truth of the foregoing facts
and for whatever legal purpose it may serve.

PAMINTUAN NANO, who is currently a resident of Milan, Italy, wants him
petitioned and live with her in the said foreign country;

That I hereby give my full consent to the desire of LILIAN


PAMINTUAN NANO to petition our son GLENN GIAN P. NANO to travel and
live with her in Milan, Italy;

That I am supportive of the planned petition because I know that


such act is for the benefit and betterment of our child GIAN GLENN and for
him to be reunited and live with her mother in Milan, Italy where greater
opportunities awaits him there;

That I am executing this affidavit to declare the truth of the foregoing facts
and for whatever legal purpose it may serve.
