I would like to appeal to your good office for the reinstatement of my Life Plan. The reason for my
lapses from the months of May to August 2021, is due to my unemployment status during this
pandemic, I acknowledge your rules and regulations, hence I humbly ask that you grant exception for my
case as this is the only time I erred from the many years of my loyalty to your service. For compensation,
I will be paying the lapses from May to August 2021 amounting of Three Thousand Pesos (3000php)
upfront today or whenever you may allow me to.

Hoping for your kind consideration and favorable response.
