Intensive Nursing

Practicum
OVERVIEW

This course deals with an actual application of theories, principles and


concept of clinical nursing practice to groups of clients in various settings, to
refine nursing skills in the different nursing basic services including
community. Emphasis is place on integrating the multiple roles of professional
nursing as a vehicle to enhance critical thinking and communication skills.

This self-learning module on Related Learning Experiences (RLE) is


based on Case-based learning wherein the use of case scenarios appropriate
for a nursing focus to achieve specific learning outcomes is emphasized. The
case is accompanied by guide questions that the students are expected to
answer by applying their knowledge from previous concepts learned as
provided by the faculty or through self-directed learning. Dependent on the
learning outcomes they are meant to fulfill, cases can be fact-driven and
deductive where there is a correct answer, or they can be context driven where
multiple solutions are possible.

This RLE module contains activities that simulate the real bedside
scenario in the hospital and in the community settings through presenting
series of case studies to cover the multiple roles of a beginning nurse on client
care, management and leadership, and research. As you accomplish this
module, may you find it helpful in honing your knowledge, skills, and attitudes
as future Titan nurse.

On completing this RLE module, you will be able to:
1. Utilize the nursing process in the care of patient.
2. Perform a holistic health assessment based on the case scenarios provided
3. Provide direct care to group of clients utilizing appropriate independent and
collaborative nursing interventions.
4. Perform administrative and other activities reflective of an actual Pediatric Ward
nurse.
5. Establish clear communication with nursing and other disciplines regarding
patient care.
6. Perform accurate and comprehensive documentation of nursing care using the
prescribed forms and formats.
7. Evaluate progress and outcomes of care.

LEARNING ACTIVITIES

▪ Supply the needed data on the Admission Sheet based on the given
scenario/case.
▪ Perform a holistic health assessment based on the case scenario provided.
Document the assessment data using BLUE font color for normal findings
and RED for abnormal findings under Review of Systems (ROS).
▪ Create a concept map using Lucid chart integrating all the concepts
(Possible risk factors, disease process/pathophysiology, signs and
symptoms, diagnostic tests, medical diagnosis, pharmacology, at least 3
nursing diagnosis, expected outcomes, and nursing interventions). Please
use the object-color-coded legend for better understanding. Refer to the
learner’s worksheet for the color-coded object.

CASE SCENARIO

Patient X, 3 year-old male, Filipino, born on Feb 1, 2018 in Sampaloc,


Manila was admitted on February 15, 2021 @ 10AM due to cough, fever,
accompanied by nausea and vomiting under the service of Dr. Kua.
Initial diagnosis: PCAP-C

Situation: You are on duty at 6-2 shift and you receive the following data
during endorsement:

• Admitted at room 1905 with 02 @ 2LPM via NC


• With PNSS 500mL to run for 8hrs @ 600mL level
• Diet for age
• Meds: Paracetamol 5mL every 4hrs for Temp >37.8 and above;
Pen-G 200,000U IV q 6hrs; Salbutamol neb 1respoule q 6hrs;
Erceflora 1 sachet TID PO, Zinc sulfate 2mL OD PO
• Latest temp: 37.9 C

History: Born full-term via CS in Manila Doctors Hospital, 2nd out of 3


children of a Jeepney driver (35 y/o) and a housewife (33 y/o). Both
parents are hypertensive and father is smoking 3 packs/day for 10 years.
Cough started 3 days prior to admission, followed by an on & off fever
thereafter. Nausea and vomiting followed after giving Paracetamol as
home remedy. 3 hrs prior to admission, temperature went up to 38.8 C.
AM shift’s nursing diagnosis is Hyperthermia. Lab/Radiological test done
February 16: XR result reveals pneumonia, WBC at 14,000, RBC &
platelets normal. Stool and urine analysis is also normal.

Focus assessment includes: RR 44cpm, irregular, shallow, breathes


through mouth, uses accessory muscle for breathing, unable to breathe
in supine position, audible breathe sounds within few feet. Auscultation
reveals bilateral course crackles on lower lobes with decreased breath
sounds at posterior area. T 38.2C, PR 125bpm, SPO2 92% with 02.
Assessment findings of all other areas must be filled-up using fictional
data BUT should be within the scope of the case scenario given.
