SPECIAL POWER OF ATTORNEY

I, __________ Filipino, of legal age, single and resident of Block 6, Lot 2, BLISS
Compound, Burgos, Quezon City, constitute and appoint ________, Filipino, of legal
age and a resident of 2436 Int I, Sta Teresita St., Sampaloc, Manila, as my true and
lawful attorney-in-fact, to appear in my behalf, in civil case docketed as 10022,
Metropolitan Trial Court, Branch 001 of Quezon City, to enter into an amicable
settlement, to submit to alternative modes of dispute resolution, and to enter into
stipulations or admissions of facts and documents.
I hereby ratify all the acts my attorney-in-fact will do pursuant to the
aforementioned.
In witness whereof, I have hereunto affixed our signature this 28 th of February
2023.
