Please be informed that the Catechetical Ministry of Sacred Heart of Jesus


Parish Church will conduct a special activity this coming March ____, 2023 from 8:00
to 11:00 in the morning this is in preparation for their scheduled first communion.

Participation in the said activity is purely voluntary in nature and does not
affect their academic standing. That the school does not hold any obligation to agree
with a monetary contribution and donation incurred during the conduct of the said
activity.

The safety of our children who will be participating in the activity is everybody’s
concern and the school shall not be held liable for any untoward incident that may
happen beyond our control.

By signing at the bottom of this document, you agree to the following:

- I have fully understood the benefits that my son/daughter/ward will


receive from participating in the said activity.
- I voluntarily gave my consent and support to my son/daughter/ward to
participate in the said activity.
