AFFIDAVIT OF LOSS
(Re: DEMAND DRAFT FROM ALLIEED BANK
PAYABLE TO LONDON SCHOOL OF MANAGEMENT)

I, JOSEFINA DELA CUESTA, of legal age, Filipino citizen and with office
address at Room 505, PPI Building, 355 EDSA cor. Gen. Tirona Street,
Balintawak, Caloocan City, after having been sworn to in accordance with law,
hereby depose and state;

1. That I am the proprietor of JO AND JAM TRAVEL, a sole


proprietor business duly established and existing under the laws of the
Philippines with office address as above-stated;

2. That on July 28, 2008 the company purchased from Allied Banking
Corporation a Demand Draft in the amount of US$2,180.73 payable to
LONDON SCHOOL OF MANAGEMENT through ROGER GARDINER
as and by way of tuition fee of one FERDINAND AROSA;

3. That such financial instrument was shipped to the payee through


FEDEX INTERNATIONAL AIR WAY through Trucking No. 8645 3989
1140 and paid through Official Receipt No. 016299 dated July 28, 2008;

4. That unfortunately, the said demand draft never reached the


intended payee/recipient and was declared lost by FEDEX;

5. That up to this time the said demand draft was never recovered,
despite diligent effort to locate the same;

6. That I am executing this Affidavit in order to attest to the truth of


the foregoing facts and for whatever purpose this may serve.

Done this 22nd day of August, 2008 at Caloocan City, Philippines.
