AGREEMENT FORM

I hereby acknowledge that I have been through all employee policies of


Teleperformance Philippines (“TPPH”) and the HMO coverage including the benefits. I understand and
agree that it is my responsibility to abide by the rules, regulations, policies, and standards and that non-
compliance may merit appropriate corrective action.

I shall also keep myself knowledgeable and updates of any changes to employee-related
rules, regulations, and policies that may take place by reading an understanding updates in small
announcements, bulletin board notices and by reading and understanding the current detailed versions
of the TPPH HR Policy Manual and the Employee Handbook, as amended by the TPPH HR Policy Manual,
which have been made available to me through the company’s intranet portal.

For any inconsistencies in the provisions between the TPPH HR Policy Manual and the Employee
