CHECKLIST ON POSITIONING AND DRAPING

Instruction: Rate the following criteria after having satisfactorily practiced the skills. Check

Positioning
 Positioning a client in good body alignment and changing the position regularly.
Draping
 It is the manner of arranging the covering in order to expose the part being
examined, treated or cleaned.
General purposes:
 To prevent muscle discomfort, undue pressure resulting in pressure ulcers,
damage to superficial nerves and blood vessels, and contractures.
 To maintain muscle tone and stimulate postural reflexes.

Performance Criteria 3 2 1 0 Remarks

I. KNOWLEDGE
1. Describe and define positioning and draping.
2. State the general and specific purposes of each
positioning and draping.
3. Enumerate special considerations.
4. State the principles/ rationale behind specific
action.
II. SKILLS
a. Erect (Standing)
1. Assist the patient to stand with either slipper on
or with bare foot on a piece of paper. Erect
position facilitates examination. Slippers or
paper placed under the feet protects them from
dirt.
 Head is erect and at midline.
 Shoulders and hips are straight and
parallel.
 Arms are comfortably positioned at each
side.
 Feet are placed slightly apart, with toes
pointed forward.
2. Untie the gown and leave the uppermost tape.

towards the front. Such procedures facilitate
examination of the body contours.
3. Place the double folded sheet around the body,
passing it under the axilla. Leave one side open
and secure it in place with safety pin. This
provides convenience and privacy during the
examination.
b. Sitting
1. Head is erect, and vertebrae are in straight
alignment.
2. Body weight is evenly distributed on buttocks
and thighs.
3. Both feet are supported on floor, and ankles are
comfortably flexed.
4. A 1–2-inch space is maintained between edge
of seat and popliteal space on posterior surface
of knee.
5. Forearms should be supported on arm rest, in
lap, or on table in front of chair.
c. Horizontal Recumbent (dorsal position)
1. Replace the top sheet with draping sheet. (The
top sheet may be used in absence of a draping
sheet). Cover the patient from the shoulders to
the foot sheet hanging loose at the sides. This
provides privacy and facilitates changes of
position.
2. Assist client to lie flat on his/ her back with legs
together, extended or slightly flexed. This
promotes better muscle relaxations.
3. Place one pillow under the head and a smaller
one may be placed under the knees. This gives
comfort and prevents hyperextension of the
knee.
4. Place arms along the side of the body
comfortably flexed on both sides. This provides
comfort and prevents interference during the
examination of the lower extremities especially
when arms are flexed.
d. Dorsal Recumbent
1. Replace the top sheet with draping sheet. The
dorsal recumbent position is indicated for
examination of the abdomen, pelvic and
perineal areas. The use of draping sheet keeps
top sheet clean and wrinkle-free.
2. Assist the client to lie on his/her back. This
position is also used to perform perineal care
and treatment.

3. Separate the legs, flex the knees so that the


soles of the feet are flat on the bed. Place the
arms in either above the head or flex with the
hands on the chest. Flexion of the knees
relaxes the abdominal muscles.
4. Place one pillow under the head. This promotes
comfort.
5. Bring the patient to the edge of the bed.
Working close to the patient prevents
overstraining of the back muscles.
6. Place the draping sheet diagonally on the
patient so that the opposite corners cover the
legs. Fold back the top corner over the chest.
Folding back the top corner over the chest
prevents inconvenience and smothering of the
patient.
7. Wrap the corner on the right foot. Do the same
with the left side. This prevents exposure of the
lower extremities and holds the drape in place.
8. Fold the lower corner of the sheet back of the
abdomen to expose the part to be examined
when the doctor is ready to do so. Having the
lower corner of the sheet loose facilitates
exposure of the part to be examined when the
doctor arrives.
e. Lithotomy
1. Insert the leggings or stockinette’s PRN.
Stockinette’s provide warmth and cover for the
legs, protect the skin from irritation.
2. Separate the legs and flex the thighs deeply
towards the abdomen. Elevate the lower legs
and support them with stirrups. Such position
allows good posture of the vulva.
3. Drawn down the buttocks to the folding edge of
the table.
4. Adjust the stirrups according to the size of the
patient. Properly adjusted stirrups prevent injury
and discomfort.
5. Raise the arms above the head or flex them
with the hands on the chest. This provides
comfort and prevents interference during the
examination of the lower extremities.
6. Drape as in dorsal recumbent.
f. Sim’s (Lateral)
1. Assist the client to lie on either side, preferably
forward. For rectal or colon procedure. The left
with the body inclined forward. For rectal and
colon procedure.

2. Extend the left arm behind the back and flex the
elbow of the right arm forward. This is position
provides comfort and prevents injury.
3. Flex the right thigh towards the abdomen with
the knee drawn up higher than the left knee that
is only slightly flexed. This facilitates the
separation of buttocks thereby allowing better
exposure of the anus.
4. Lay out the draping sheet as in horizontal
recumbent position. Fold back and or gather a
side of the sheet to expose the area to be
examined. Proper draping provides comfort and
privacy.
g. Prone
1. Help the client assume the horizontal
recumbent position. This is preparatory to the
desired position.
2. Assist him turn over unto the abdomen. This is
for back and spinal examination and
procedures.
3. Turn the head to one side, this is done for
comfort, convenience, and prevents smothering.
4. Place the arms at the side or flex or extend
upwards. This provides comfort and prevents
interference during examination of the lower
extremities especially when arms are flexed.
5. Allow the feet to hang over the edge of mattress
or support on a pillow high enough to keep the
toes from touching the bed. This prevents
plantar flexion. Proper draping provides comfort
and privacy.
h. Fowler’s Position
1. Place the client in a horizontal recumbent
position.
2. Elevate the head of the bed to approximately 45
degrees angle. This is to provide comfort and
facilitates various procedures.
3. Flex the knees slightly and support them with
knee rolls. Flexion prevents hyperextension of
the knees and prevents slipping from the bed.
4. Drape as in dorsal recumbent. Proper draping
provides comfort and privacy.
i. Trendelenburg (Shock Position)
1. Assist the client to a horizontal recumbent
position. Trendelenburg position is done for
certain types of shock, surgical procedures, and
postural drainage.

2. Elevate the foot of the bed so that the lower


tuck is higher than the head and shoulders.
3. Support the shoulders and knee. The weight of
the body is pulled downwards by gravity.
Supports will prevent the patient from slipping
out of the bed.
4. Drape as in horizontal recumbent. Proper
draping provides comfort and privacy.
j. Knee-Chest (Genupectoral)
1. Place the client on a prone position. This is
preparatory for assuming the desired position.
2. Assist to kneel with the knee slightly separated.
3. Bend forward so that the chest is resting on the
bed and the thighs are perpendicular to the
legs. This is for the rectal and vaginal
examination and as a form of exercise for some
gynecological conditions.
4. Turn the head to one side and place the arms
either above head or flex at the elbow and rest
along the side of the head. This is for support,
convenience, and prevention from smothering.
5. Drape the patient properly so that the only area
to be examined is exposed. Proper draping
provides comfort and safety.
