To Whom It May Concern;

We would like to inform you that permission is hereby granted to our son/daughter
_____________________________________ to join the Criminology Internship Training Unit (CITU) Acceptance and we
will follow the conditions stated below.
1. The said activity will have a contribution amounted to One-Hundred Fifty Pesos (₱150.00).
2. The said activity will have a five (5) days duration training starting from June 21 to June 25, 2023.
3. On June 24, 2023 he/she will stay at LSSTI School premises for their overnight activities following their
acceptance day.

We understand that the said conditions are given for the sake of my son/daughter to join the Criminology Internship
Training Unit (CITU) Acceptance. Thus, we are aware that certain activities are one of the requirements of their course and
they only prepare my son/daughter for the near future if he/she will choose the law enforcement profession.

However, if despite all efforts taken, untoward incidents beyond their control occur, we will not hold Lanao School of
Science and Technology, Inc., Instructors and the Trainers liable.
